As a Senior Physiotherapist within our fast-paced service, you will provide specialist assessment, treatment and rehabilitation to individuals who may otherwise require hospital admission. Working directly with patients in their own environment, you will support recovery, promote independence and help them remain safely within their community.

You will be part of a supportive multidisciplinary team responding rapidly to changing patient needs and delivering compassionate, person-centred care. Working closely with health, social care and community colleagues, you will contribute to integrated care plans and positive patient outcomes.

Main duties of the job

As a Senior Physiotherapist within the Brent UCR team, you will deliver high-quality assessment, treatment and rehabilitation across community settings, including patients' homes, care homes and residential facilities. Working as part of a multidisciplinary team, you will support admission avoidance, facilitate timely hospital discharge and promote independence and recovery.

• Delivering holistic assessments and evidence-based physiotherapy interventions tailored to individual needs.
• Prescribing and advising on equipment and adaptations to support safe and effective care at home.
• Contributing to care planning, risk assessment and clinical decision-making.
• Supporting, supervising and developing junior colleagues and students.
• Building effective partnerships with patients, carers, families and multidisciplinary colleagues.
• Contributing to a flexible, responsive and person-centred service that meets the needs of the local community.
