This role will be employed through Oxleas on behalf of the Bexley system to support neighbourhood delivery. The role will lead on the implementation of the priorities as set out in the Bexley Wellbeing Partnership Neighbourhood Plan and support the development of the three Local Care Networks with their delivery plan – with a particular focus on improving outcomes and reducing health inequalities.
This role will be responsible for the leadership, management and performance of the Bexley Integrated Neighbourhood Teams, comprising primary care, Oxleas physical and mental health services, adult social care and voluntary sector partners.
The post holder will lead the delivery of high-quality, integrated health and social care services that support people to remain independent and receive care closer to home
Main duties of the job
- The post holder will provide strategic and operational leadership and oversight of neighbourhood multidisciplinary teams, ensuring delivery of integrated health and social care services. · Ensure delivery of safe, effective, person-centred services through robust care pathways and partnership working.
- Hold services to account for performance against agreed quality, access, activity and outcome measures.
- Lead service improvement and transformation programmes that improve patient outcomes, experience and operational efficiency.
- Identify, manage and resolve complex operational issues and risks, implementing sustainable solutions in partnership with clinical and managerial leaders.
- Coordinate demand, capacity and resource planning across neighbourhood partners to support effective service delivery, patient flow and system resilience.
- Provide operational assurance to senior leaders regarding service performance, delivery of neighbourhood objectives and implementation of agreed operating models.

- Management responsibilities
- Lead the delivery of integrated, place-based care across multidisciplinary teams, ensuring seamless working across health, social care and voluntary sector partners.
- Work collaboratively with communities and system partners to deliver a neighbourhood model focused on prevention, independence and population health outcomes.
- Develop and embed a person-centred, outcome-focused team culture aligned to population health approaches.
- Ensure staff access mandatory training and continuing professional development aligned to service and system priorities.
- Maintain effective communication, engagement and feedback mechanisms through governance and team processes.
- Manage budgets and resources effectively, ensuring financial control, cost efficiency and sustainable service delivery.
- Ensure optimal deployment of workforce and resources to meet demand safely and efficiently.
- Deliver against performance, quality and contractual targets, using data to monitor trends, identify risks and drive improvement.
- Use data and insight to support demand management, capacity planning and service optimisation.
- Develop and maintain robust governance systems, ensuring high standards of risk management, record keeping and safe service delivery.
- Ensure safeguarding responsibilities are embedded and consistently applied for adults and children.
- Lead the management of complaints, incidents and risks, ensuring timely resolution and embedding learning into practice.

- Contribute to policy development, service improvement and transformation across the neighbourhood and wider system.
- Ensure compliance with statutory and organisational requirements, including health and safety, data protection, equality and safeguarding.
- Provide line management and leadership for designated staff, including recruitment, appraisal, performance management, capability management and professional development.
- Exercise delegated authority for operational and financial decision-making within agreed organisational limits, ensuring appropriate use of resources and delivery of agreed objectives.
- Leadership
- Provide visible, inclusive leadership across integrated neighbourhood services, ensuring alignment to strategic priorities and organisational objectives.
- Lead the development of service objectives and transformation plans, with clear outcomes and measurable impact.
- Build and maintain strong system relationships with partners, commissioners and stakeholders to support integrated working.
- Champion co-production and engagement with clinicians, staff, service users and carers in the design and delivery of services.
- Embed a culture of continuous improvement, audit and evidence-based practice aligned to NICE guidance and best practice.
- Work collaboratively with senior leaders to maximise workforce contribution, governance and service delivery.

- Clinical/Professional
- Provide leadership alongside clinical and professional leads to ensure neighbourhood services are safe, effective, person-centred and aligned to agreed clinical and professional standards.

Communication
Communicate complex and sensitive information clearly to staff, partners, residents and senior stakeholders, adapting style and approach to different audiences.
Establish effective communication and engagement mechanisms across neighbourhood teams, primary care, adult social care, voluntary sector partners and wider system stakeholders.
Use negotiation, facilitation and influencing skills to support collaborative decision-making, resolve issues and maintain shared focus on agreed outcomes.
Prepare and present clear reports, briefings and updates for governance meetings, senior leaders and partnership forums.
- Influence, negotiate and build consensus across organisational boundaries where priorities, resources or professional perspectives may differ.
- Policy Development
Lead the development, review and implementation of neighbourhood-specific policies, protocols and operating procedures, ensuring these support integrated working across partner organisations.
Interpret relevant national, regional and organisational policy guidance, translating it into practical operational arrangements for neighbourhood teams.
Support the development of memoranda of understanding, service agreements and escalation processes where these are required to enable safe, effective cross-organisational working.
- Lead implementation of national policy, integrated care strategies and local priorities, translating these into operational delivery models across neighbourhood services.
Research
Use evidence, benchmarking, population health intelligence and service evaluation to inform decision-making, service redesign and continuous improvement.
Support research, innovation and best practice initiatives that contribute to improved outcomes, experience and value across neighbourhood services.
