Job overview

To provide a specialist speech and language therapy service as part of the Community Neurological Rehabilitation Team for adults within the tri-borough area of Hammersmith & Fulham, Kensington & Chelsea, and Westminster. The post holder will work in collaboration with the community teams within CLCH, other health care professionals, statutory and voluntary agencies to deliver excellent standards of patient care.

Main duties of the job

  • The post holder will be based within one of the three MDT neurorehabilitation teams, and provide specialist assessment and treatment of clients with a wide range of acquired neurological conditions with swallowing and/or communication impairments. The post holder is also part of the Early Supported Discharge Team (ESD) for Stroke. This includes the opportunity to work as a service coordinator, triaging and managing ESD referrals from acute site partners.
  • The post holder will be an integral member of the community neuro-rehabilitation team based in the specified borough, acting as a resource to other medical, nursing, therapy, social services and voluntary agency staff. They will build and maintain key relationships with referrers to the service (including GPs, acute hospitals, bedded services, Care or Neuro navigators) to facilitate appropriate referrals through promoting understanding of the role of the speech and language therapist within the multi-disciplinary team (MDT).
  • The role covers a variety of settings including clients’ homes, out-patient clinics, residential and community facilities. The post holder would be expected to provide advice, training and support to nursing home staff, students, therapy assistants and others.

Person specification

Experience

Essential

  • Two years minimum of Adult postgraduate experience or more working with a wide range of adults with communication and/or swallowing impairment
  • Experience of working independently with Adult clients with dysphagia
  • Experience of training others
  • Team work in a multidisciplinary context including working with carers and other professionals

Desirable

  • Experience of providing SLT intervention to adults requiring intermediate care and or palliative care

Skills & Knowledge

Essential

  • Excellent Interpersonal skills including observation, listening and empathy
  • Able to work well in a team setting and utilise a client centred, goal setting approach to rehabilitation and intervention, in partnership with clients and carers
  • Good working knowledge of appropriate range of assessments and interventions for clients with language, communication and swallowing impairments. An ability to compare and contrast relative benefits to individual clients and of the issues surrounding disability and rehabilitation
  • Knowledge of legislation and NHS policy documents relevant to the area of practice (e.g. The National Service Framework for Older People, RCP Guidelines on Stroke.)
  • Good understanding of the roles of other professionals (relevant to the client group)
  • Knowledge of the principles of clinical governance and audit

Education/Qualification

Essential

  • Degree and the Royal College of Speech/Language Therapists Licence to Practice.
  • Registered member of HPC and RCSLT
