Oxleas NHS Foundation Trust is seeking an experienced and motivated Registered Nurse to join the Primary Care team at HMP Bristol on a bank basis.

This is an opportunity for a nurse with a strong background in practice nursing, district nursing, long-term condition management, wound care and physical health to provide high-quality, evidence-based primary care to people in prison.

The successful candidate will complement the existing nursing workforce and provide additional specialist primary care capacity, supporting the delivery of care that is equitable with that available in the community.

Working within a multidisciplinary healthcare team, you will undertake assessment, treatment, monitoring and review of patients with a range of acute and long-term health conditions. You will be expected to work autonomously within your scope of practice, while recognising when escalation or multidisciplinary input is required.

Please note: We are looking for registered nurses at band 5 or 6, banding to be discussed at interview and is based on experience/competencies.

In order to work as a bank only worker you will need to have completed preceptorship training or obtained 6 months post qualification experience.

Main duties of the job

Key responsibilities:

The post holder will:

• Undertake comprehensive nursing assessments and develop, implement and evaluate individualised care plans.
• Provide evidence-based management and review of patients with long-term conditions.
• Undertake chronic disease reviews and contribute to the management of patients with complex physical health needs.
• Support the management of conditions including diabetes, cardiovascular disease and hypertension, respiratory disease including asthma and COPD, epilepsy, chronic kidney disease and other long-term and complex physical health conditions.
• Undertake assessment and management of wounds, including chronic wounds and leg ulcers.
• Provide leg ulcer care, wound assessment, dressing selection and application, and compression therapy where appropriately trained and competent.
• Recognise deterioration in patients with long-term conditions and initiate appropriate escalation and referral.
• Undertake physical health observations, clinical assessment and appropriate investigations within scope of practice.
• Provide health promotion, prevention and patient education.
• Support screening and preventative healthcare programmes.
• Contribute to medication monitoring and medicines optimisation within professional competence and local procedures.
• Liaise effectively with GPs, advanced practitioners, pharmacy, specialist nurses and other members of the multidisciplinary team.

Specialist skills and experience:

We are particularly interested in nurses who have experience in:

- Practice nursing or primary care. - District nursing or community nursing. - Long-term condition management. - Diabetes management and review. - Cardiovascular risk management and hypertension. - Asthma and COPD management. - Epilepsy and seizure management. - Leg ulcer assessment and management. - Chronic wound assessment and wound management. - Compression therapy. - Physical health assessment. - Health promotion and preventative healthcare. - Working with patients with complex or multiple health needs.

Experience of working within a prison or other secure environment would be advantageous but is not essential. Appropriate induction and support will be provided to nurses who are new to the secure environment.

You will be an experienced Registered Nurse who is confident in your clinical decision-making and has a particular interest in primary care, long-term conditions and wound management.

You will be able to work independently while recognising the importance of effective multidisciplinary working. You will have excellent assessment and communication skills and be able to build therapeutic relationships with patients from a diverse and often complex population.

You will demonstrate a commitment to delivering safe, effective and person-centred care and will be able to adapt your practice to the unique requirements of a secure healthcare environment.

Address History:

5 years address history will be needed.

Applicants that are not UK Passport holders who provide less than 5 years UK address history will need to provide a Police Certificate which must be in English from where they resided previously.

Applicants who are UK Passport holders who have lived abroad for a period of more than six months during the last three years will need to provide a certificate of good conduct or an overseas police check in English from the countries resided in or visited.
