Main duties of the job
We are seeking to recruit an asthma clinical nurse specialist (CNS) who will be working within an integrated neighbourhood model supporting the delivery of a pilot to identify and manage children and young people (CYP) at high risk of asthma exacerbations. As a Clinical Nurse Specialist, you will act as a lead specialist within paediatric asthma across community and primary care settings, contributing to an integrated neighbourhood team (INT) approach. This includes working collaboratively with GP practices, paediatricians, pharmacists and wider partners to proactively identify, review and manage CYP through population health management and risk stratification approaches. You will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, including development of policies, protocols and guidelines and contribute to service developments in collaboration with multidisciplinary colleagues. Also ensures the safety and wellbeing of services users and always working in a manner that promotes dignity and human rights through the adoption of person-centred care principles. You will benefit from excellent support from the existing CNS team and Asthma consultants & we will offer you a comprehensive range of experience and development opportunities.
