Job overview

The Bank Advanced Clinical Practitioner (ACP) will provide advanced clinical expertise and leadership within the Single Point of Access service, helping to reduce unnecessary hospital admissions, support admission avoidance, and facilitate timely hospital discharge. The post holder will undertake advanced clinical assessment, diagnosis, treatment planning and care coordination for patients with acute, undifferentiated and long-term conditions. They will work autonomously, provide clinical leadership, support service development, and contribute to education, mentorship and quality improvement initiatives across Adult Community Services.

Main duties of the job

The post holder will act as a first point of clinical contact, undertaking advanced assessments, clinical triage and decision-making for patients referred into the service.

They will initiate investigations, interpret diagnostic results, formulate differential diagnoses, make referrals to appropriate services, prescribe independently, and coordinate care plans.

The role also includes preventing inappropriate hospital admissions, supporting discharge planning, safeguarding vulnerable individuals, managing complex caseloads, and working collaboratively with multidisciplinary and partner organisations to deliver high-quality patient-centred care.

The ACP will also provide clinical leadership, supervision, education and support to colleagues and students.

  • Provide advanced clinical assessment, triage, diagnosis and management of patients with undifferentiated and complex health conditions.
  • Independently prescribe and review medications within own scope of practice.
  • Initiate and interpret diagnostic investigations and make appropriate referrals.
  • Support admission avoidance and facilitate safe and timely hospital discharge.
  • Lead and contribute to service quality improvement, clinical audit and governance activities.
  • Provide clinical leadership, supervision, mentorship and education to staff and students.
  • Work collaboratively with multidisciplinary teams and external partners across the healthcare system.
  • Maintain accurate clinical records and ensure compliance with professional, organisational and legal standards.
  • Support service development, innovation and implementation of evidence-based practice.
  • Promote safeguarding, patient safety and high standards of patient care at all times.
