Bank Community Diabetes Specialist Nurse

Job summary

We are recruiting a Bank Band 7 Community Diabetes Specialist Nurse to work within East Coast Community Healthcare Specialist Diabetes Nursing Team.

The post holder will need to demonstrate a comprehensive knowledge base and experience in relation to managing type 2 diabetes in the community.

The post holder will contribute to the delivery of specialist nursing care, case management and provide direct patient care, acting as an expert practitioner to optimise patient health, provide education for self-care and complex management of patient pathways.

Post locations - Remote/home, GP surgery clinics, ECCH bases including Yare House, Gorleston / Kirkley Mill Health Centre, Lowestoft / Northgate Hospital, Great Yarmouth.

Main duties of the job

To provide clinical leadership, operational management and professional support to the ECCH Community Diabetes Specialist Nursing Service. Develop and deliver the integrated diabetes service across Norfolk & Waveney East locality.

To work with other members of the multidisciplinary team to promote diabetes health and effective clinical management. To work autonomously at a level appropriate to clinical competence and within the scope of professional practice.

To provide specialist training sessions for staff across ECCH and all external stakeholders.

To undertake with others, continuous evaluation of the service to ensure it is fit for purpose and increases access and choice .

To contact, assess, monitor and support patients/clients and their families ensuring holistic health needs assessment as well as medication adherence, treatment and follow up.

Liaise with Endocrinologists, GPs and community staff as required.

Organise/attend/participate in diabetes team meetings as required.

To advise on treatment plans for the patient.

To develop individual care plans in partnership with clients based on a shared assessment of need.

To formulate and review/update diabetes policies.

Attend in service training courses and study sessions as appropriate to maintain own competencies.

Plan, implement and evaluate diabetes training to students and other members of ECCH staff.

Job responsibilities

Maintain contemporaneous and accurate written and electronic records in partnership with clients and in accordance with policies and procedures.

Take responsibility for management of team members as required.

To lead on tasks as delegated by line manager.

Write reports and participate in audit on service delivery, as required for manager and ECCH Board.

Devise local protocols for specialist working and ensure implementation as required.
