We are looking for a highly skilled and motivated Population Health Management (PHM) Operational Lead to join the South & West Hertfordshire Health and Care Partnership (HCP). This exciting senior role, is ideal for someone with strong analytical expertise, excellent relationship building skills, and a passion for using data to improve population health and reduce inequalities.

As the PHM Operational Lead, you will play a pivotal role in developing PHM capability, embedding analytics into clinical and operational workflows, and supporting the creation of insight-driven system-wide transformation. You’ll help position the HCP at the leading edge of PHM, shaping decision making and driving improvements in outcomes, value and care delivery.

Main duties of the job

Lead the development of operational PHM capability within neighbourhood teams.

Build reporting infrastructure to measure outcomes.

Produce insight packs, ad hoc analytical briefings and predictive modelling to support strategic decisions.

Embed PHM tools into clinical and operational workflows.

Provide analytical insight to inform resource allocation, financial modelling and business case development.

Support the adoption and improvement of PHM tools.

Work with partner analytics teams, including local authority public health, adult social care and children’s services.

We’re looking for someone with: Advanced analytical and statistical skills, with experience in modelling, forecasting, and visualisation.

Strong experience working with clinicians and operational managers to influence decision making.

Demonstrable experience in leading analytical work across complex systems.

Excellent interpersonal, communication and influencing skills.

Expertise in presenting complex information clearly to senior stakeholders.

Detailed job description and main responsibilities

In this role, you will drive the use of population health data and tools to shape strategic decision making, support transformation, and strengthen the system’s ability to improve outcomes for local communities.

You will be: Leading the development of PHM capability across the HCP, including the training, mentoring and upskilling of colleagues.

Developing and maintaining reporting infrastructure for neighbourhood and place-based outcomes.

Producing insight packs, analytical briefings and predictive modelling.

Providing insights to inform decision-making on system resource allocation and commissioning options.

Tracking and validating system-wide outcomes.

Integrating risk stratification, predictive modelling and PHM tools into clinical and operational workflows.

Broadening organisational understanding of PHM methodologies and promoting best practice.

Working with trust BIU teams to ensure PHM principles and tools are adopted and effectively utilised.

Building strong relationships with system partners.

Helping to build alignment and understanding across different organisational cultures, priorities and analytical needs.
