A new and exciting opportunity has arisen for an influential leadership position to deliver clinical services operating within a Locality Hub model. The Locality Hub is seen as a key component in delivering Neighbourhood Health as per the NHS 10 Year Plan. BCHC, as the lead for the Community Care Collaborative has been influential in the 10 Year Plan, receiving national recognition for its collaborative approach across health and care sectors, making a difference to local communities and the care they receive.
Each Locality Hub: of which there are 5 designated for Birmingham, has four currently in progress to become established by the end of September 2025. East Locality Hub (Washwood Heath Health & Wellbeing Centre) has been operational for over 12 months and has established the framework for delivering Neighbourhood health across the City. The hubs core components consist of a Care Co-ordination Centre, associated Integrated Neighbourhood Teams and designated clinical pathways to provide same day emergency care to its local community, thereby preventing unnecessary admission to hospital. This model of care is dependent on complex system data and the interpretation of this, and subsequent operational activity are core to this role.
The Service Manager for Locality hubs is a critical leadership role and will have operational oversight of services being delivered. It will play a key role in shaping and ensuring delivery of designated care services within the Locality Hub via its collaboration with Acute, Mental Health, Primary Care and Voluntary Sector partners. This truly innovative and integrated model of care is seen as a real opportunity to make a difference to its respective local community and the care they receive.
This role demands a dynamic, inclusive and engaging leader who can continue to motivate the passionate caring teams, and drive service transformation, operational performance and excellence, with workforce engagement across complex care environments. The post holder will lead on resource allocation, demand and capacity planning, performance improvement, and service development, ensuring that services are responsive to local needs and aligned with wider system priorities.
