We are looking for a full time or part time Non-Medical Prescriber. An opportunity has arisen to join the REACH team as a Non-Medical Prescriber; supporting people who use drugs and alcohol to stabilise, work towards recovery and build positive and fulfilling futures for themselves.

As a prescriber you will work with an allocated cohort of service users from a specific area of Dorset (various options available) and be part of an integrated team of drug and alcohol professionals. Clinical leadership is provided by an AWP Consultant Addictions Psychiatrist.

We are looking for a skilled Non-medical prescriber with experience of working within a drug and alcohol treatment setting and a passion for supporting people towards the best possible outcomes. Main duties of the job

To work as an Independent Non-Medical Prescriber in partnership with the service user and act as a specialist in Non-Medical Prescribing for service users with substance misuse needs.

To work as an experienced member of staff in the Reach Team. Provide and develop a specialist drug and alcohol service to service users, including the assessment of treatment and care needs, and the planning, implementation and evaluation of care to meet needs and contributing to the evaluation of care across the REACH prescribing service with a variety of settings where specialist prescribing is available.

Senior Nurse prescriber for service users working with the Reach care coordinator and other members of the multidisciplinary REACH team and other agencies involved in the care of individual service users.

Contribute to Integrated Governance processes, including the continuing development, improvement and monitoring of service provision and performance, and monitoring standards of care, through data collection and audit processes. Liaise with, and provide support, advice and training as appropriate to REACH colleagues/services and agencies involved in provision of services to people with drug and/or alcohol problems.

Contribute to the supervision of identified staff, and provision of training for students of various disciplines, as appropriate.

• To undertake comprehensive assessments of service users referred to the team, both urgent and routine, including specialist assessments of clients requiring non-medical prescribing, ensuring that a full health and social needs assessment, including medical history and medication history, is undertaken and implemented where required.
• To establish diagnosis and prescribe accordingly, discussing all treatment options (including non-pharmacological treatment) and potential side-effects with each service user.
• To ensure that record keeping is both up to date and accurate adhering to Trust, NMC and good practice guidelines, including clear documentation of medications discussed their effects and risks.
• To establish, plan and implement Clinical Management Plans (CMPs) where appropriate and review at least annually.
• To request formal diagnostic tests in line with Trust, NICE and Department of Health guidelines.
• To assess and review the service user’s progress with their care coordinator.
• To prescribe and change medications within area of speciality and competence.
• To formulate, implement and evaluate individual care plans and nursing treatment plans with the involvement of all parties in the care of the service user.
• To work within Trust ‘Medicines’ and ‘Non-Medical Prescribing’ Policies.
• To establish and maintain good liaison with GP practices and other mental health services in the area, including sharing prescribing information and rationale.
• To maintain comprehensive case records in line with Trust requirements, including recording medications prescribed.
• To ensure that all prescribing decisions are communicated.
• To be the lead Drug and Alcohol nurse prescriber supporting the care of service users within the framework of the Care Programme Approach [CPA]
• To undertake risk assessments and management plans.
• To monitor and evaluate the quality of care given to service users.
• To offer carers an assessment of their needs.
• Prepare reports for tribunals, case conferences as required.
• To ensure the Trust Policies in relation to CPA, Risk Management and Confidentiality are strictly adhered to.
• To maintain an holistic approach with service users and work with them in all aspects of their lives in a variety of settings.
• To participate in the Reach SPOC Duty System to provide urgent response and advice to service users as required.
