The post holder will be able to provide expert clinical expertise, operational management, to the Virtual Wards (Hospital @Home) Model within Birmingham Community Healthcare (BCHC) NHS Foundation Trust.

The purpose of the role is to work across the system in a variety of city-wide settings but not exclusive to the community and inpatient units, and to in-reach into secondary care, to identify and assess patients, to prevent avoidable hospital admissions and facilitate early supportive discharge.

The work will be undertaken collaboratively with Secondary and Primary Care, and Community Services, including Urgent Care Response, Early Intervention Team (EICT), and Specialists Services.

The post holder will work attend integrated multidisciplinary teams (MDT’s) to provide advice and support and agreed data to support the Virtual Ward (Hospital @Home) Model.

Main duties of the job

  • Work in accordance with the NMC / HCPC Code of Practice
  • Be conversant with all BCHC NHS FT policies and procedures and ensure they are correctly implemented
  • Ensure evidence-based care is practiced and promoted at all times.
  • Manage own time effectively and prioritise workload.
  • Facilitate teaching and supervision for less experienced members of staff.
  • Promote health by collaborative working with patients and their relatives.
  • Provide support to the multidisciplinary team to enable patients to make better choices for their health and that of their families, undertaking health promotion and contributing to the prevention of adverse effects to health and wellbeing.
  • Contribute to the review and development of integrated care pathways and clinical guidelines to ensure a standardised approach to care.

  • Facilitate and empower staff development and empowerment of all team members to perform to high standards and to innovate.
  • Act promptly when there are complaints in line with Trust policy.
  • Highlight circumstances giving cause for concern in relation to patient care, health and safety and follow Trust procedure regarding the reporting of incidents.
  • Support with staff knowledge and skills to promote equality and diversity and address inequalities, both in employment and service delivery.
  • Ensure supportive staff management arrangements are in place and carry out personal development reviews for direct reports.
  • Ensure all staff in team/s have annual PDR’s resulting in specific objectives and effective personal development plans in line with the Knowledge and Skills Outline Framework.

Person specification

Experience

Essential

  • Significant experience as a registered health professional working in ED, frailty, community or older adult setting or care
  • Significant experience of a multidisciplinary approach to care.
  • Experience of working in a telephone / remote triage service

Skills/knowledge

Essential

  • Skills to organise team workload and prioritise.
  • Awareness of legislation affecting the role including equal opportunities law.
  • Ability to evidence application of utilising evidence-based practice. Work autonomously
  • Comprehensive assessment skills and knowledge of the needs of the older person including risk management.
  • Knowledge of current legislation, particularly those affecting community services.
  • Computer literacy skills.
  • Knowledge and application of the NMC/ HCPC Code of Practice.
  • Understanding of the principles and application of Clinical Governance
  • Excellent verbal and written communication skills.
  • Understanding of the complex needs of a socially and culturally diverse population.
  • Understanding of legislation related to Safeguarding Vulnerable adults and children.

  • Hold a full, valid, UK driving licence and have access to a vehicle
  • Required to work flexible hours including weekends and Bank Holidays as part of a rota.
  • Ability to work across various sites across the community, inpatient and acutes hospitals in Birmingham

  • Registered Nurse Level 1 (Adult)/ AHP.
  • Advanced Decision Making
  • Independent Prescribing
  • Mentoring or teaching qualification.
  • Evidence of up-to-date post registration education relevant to the role.
