Job Summary

The post holder will be responsible for the provision of Clinical Pharmacy Services to ensure safe, clinically effective and cost effective use of medicines.

The post holder will be a member of the pharmacy team providing a clinical pharmacy service to the Belfast Health and Social Care Trust. In order to meet service needs and Trust objectives the post holder may either:

Be based on a fixed ward/area as a clinical pharmacist or

Gain rotational experience as a clinical pharmacist across medical, surgical and other core clinical specialties. Rotations will be of defined structure to offer clinical experience whilst balancing with service needs.

Periodically for business continuity purposes and ensuring service delivery you will be required to work, within your sphere of competence, on other pharmacy sites within the Trust.

Key Duties & Responsibilities

For each of the following, the post holder will

Clinical and technical

Provide clinical pharmacy services in a range of specialties, including wards, dispensaries and outpatient clinics.

Where appropriate, and with the appropriate qualifications, to be an Independent Pharmacist Prescriber within an agreed area. Practice as an Independent Pharmacist Prescriber will be within the scope of individual prescribing activities, as approved through the Trust Non-Medical Prescribing (NMP) application and registration process.

Demonstrate and communicate specialist clinical knowledge.

Assist in promotion of safe, effective and timely prescribing and administration of drugs.

Promote prescribing initiatives across the Belfast Trust.

Obtain an accurate medication history on admission and perform medicine reconciliation.

Provide a comprehensive prescription monitoring service including

identification and prevention of adverse drug reactions, drug related events and drug interactions

therapeutic drug monitoring

utilising information from patient, notes and blood results to ensure safety and efficacy of medication

rationalisation of medicines

Manage Patients Own Drugs (PODs) where appropriate.

Clinically check discharge prescriptions at ward level and ensure timely discharge information is available to primary care healthcare professionals.

Provide medication counselling and education to patients and their carers where appropriate.

Provide medicine related information and advice to other healthcare professionals.

Contribute to the development of protocols, policies, patient group directions and new drug requests.

Advise on and support the safe and secure storage of medicines at ward level in compliance with legislation and the Trust’s Medicines Code.

Actively support the principles of risk management.

Analyse, review and report on drug usage and expenditure data.

Identify new prescribing practices that may impact on drug expenditure (horizon scanning).

Identify areas where drug expenditure may be minimized and neg negotiate/ influence practice within the multidisciplinary team to realize these cost improvements.

Support the development of clinical pharmacy services as set by national initiatives including the Northern Ireland Standards for Clinical Pharmacy.

Participate in audits and monitoring of pharmacy services and medicines management through data collection, evaluation, interpretation, reporting and dissemination of results.

Provide, on a regular basis, statistical information relative to activity levels including the recording of clinical interventions and to meet targets and objectives agreed with the Pharmacy Services Manager and Clinical Pharmacy Co-ordinator/ Pharmacy Team Leader.

Participate in medicine related clinical audit, clinical trials and research and development (across Trust, site or designated area accordingly).

Management and Leadership

Review individually, at least annually, the performance of any immediate subordinate staff and provide guidance and training where appropriate.

Participate on recruitment and selection panels for junior pharmacy staff.

Plan and organise own daily work according to agreed objectives and time frames.

Educational Responsibilities

Act as a tutor or mentor for staff undertaking, departmentally agreed postgraduate studies.

Support the development and training of pharmacy staff including junior and Foundation Training Year pharmacists, undergraduate students and technicians.

Provide education and training to medical, nursing and other healthcare professionals and staff as appropriate.

Undertake personal development, identifying own training needs and maintaining CPD portfolio.

Participate in training and education as required by Trust and department mandatory requirements.

Other Duties

Maintain required standards of appearance and dress.

Maintain all necessary records and documentation according to pharmacy and trust policies.

Participate in dispensary duties.

Ensure the safe and secure storage of medicines in accordance with all medicine legislation and Trust security policies and to participate in Controlled Drug quarterly checks.

Participate in the BHSCT Extended Hours Service.

Participate in the BHSCT On-call service.

Participate in the Saturday and Bank Holiday Rota.

Pharmacy Services are provided from a number of locations across the Belfast Trust and you will be required to work from any of those Trust sites during the Saturday, EHS or On-call service.

Support the Trust in its drive for seven day working to best suit the needs of our patients.

Act as a Responsible Pharmacist when required.

Infection Prevention and Control

The Belfast Trust is committed to reducing Healthcare associated infections (HCAIs) and all staff have a part to play in making this happen. Staff must comply with all policies in relation to Infection Prevention and Control and with ongoing reduction strategies. Standard Infection Prevention and Control Precautions must be used at all times to ensure the safety of patients and staff.

Cleaning hands either with soap and water or a hand sanitiser at the appropriate times (WHO ‘5 moments’);

Using the correct ‘7 step’ hand hygiene technique;
