Hammersmith & Fulham a great place to grow old.   The Community Independence Service (CIS) has an opportunity for a Band 6 Physiotherapist to join our rehabilitation team.

The Community Independence Service (CIS) provides planned and unplanned nursing and therapy interventions for older residents who have had a recent deterioration in their physical health and is run jointly between West London NHS Trust and CNWL Foundation Trust across the three inner London Boroughs.

We are achieving excellent results in community rehabilitation and admission avoidance for the people of Hammersmith and Fulham and are part of a wider community independence service offered in Westminster and the Royal Borough of Kensington & Chelsea

Main duties of the job

As a Band 6 Physiotherapist, you will provide an integrated Health & Social Care service for people at home who are at risk of being admitted to hospital unnecessarily, providing Community Rehabilitation, Rapid Response, Reablement and Supported discharge.  You will be working closely with our acute partners, such as Imperial College Healthcare NHS Trust and Chelsea & Westminster Hospital NHS Foundation Trust.

We will connect you with peers across the tri borough of Hammersmith and Fulham, Westminster and Kensington and Chelsea to support you and to work on service improvement partnerships.

We will ensure you are able to follow the clinical and leadership development priorities identified in your personal development and appraisal plan.

We will equip you for mobile working and actively support flexible working practice when required and appropriate for the service.

Essential

  • Understanding and ability to apply relevant knowledge and skills required to act as an effective professional clinician and adhere to your relevant code of conduct/practice
  • Ability to plan and use assessment methods that are appropriate for complex needs, and use processes of reasoning that are based on available evidence including national guidelines such as NICE and other local and professional policies
  • Ability to follow processes of reasoning which balance additional information against the overall picture of the individual’s needs to confirm or deny developing hypotheses.
  • Knowledge and understanding of the role of intermediate care in supporting a timely discharge
  • Knowledge and ability to use Microsoft Office suite for various administration tasks, including
  • Working with spreadsheets document writing and email

  • Knowledge and ability to use patient record systems such as SystmOne or similar.

Essential

  • Excellent communication skills and fluency in written and oral communication
  • Ability to maintain accurate, timely, concise and legible clinical records both electronic and handwritten which adhere to the Trust policies
  • Ability to talk to people about sensitive issues in a supportive manner but also challenge others assertively where necessary

Experience

Essential

  • Previous experience of working in the UK or a similar health economy (acute or community) as a physiotherapist.
  • Previous general experience to include the following: · Older people/frailty assessment and rehabilitation · Medicine and general surgery · Orthopaedics · Falls · Dementia · Long-term conditions
  • Experience teaching juniors and assistants

Desirable

  • Experience working as an autonomous practitioner in a community setting
  • Previous general experience to include the following: · Community working · Neurology · Pain management
  • Experience teaching students

Qualifications

Essential

  • Degree / Diploma Physiotherapy
  • Registration with HCPC as a Physiotherapist

Desirable

  • Member of CSP and or involvement in professional network
  • Post Graduate MSc Degree
  • Post-registration courses e.g.: · Manual handling · Goal setting · Frailty and older people’s rehabilitation · Student placement facilitator
