
Job summary
<p>As a Band 6 Physiotherapist, you will join our Early <span class="single_desc_int_gb_word">Supported Discharge (ESD) team based at Gravesham Community Hospital in Gravesend</span>. This is a rewarding community role, <span class="single_desc_int_gb_word">supporting patients with neurological conditions, including stroke, to transition safely from hospital to home. You will play a key role in delivering high quality rehabilitation within patients own environments, helping them regain independence and confidence. This is a varied role where you can follow the patient journey, making a real and measurable impact on recovery outcomes</span>. You will be part of a friendly and experienced multidisciplinary team, working closely with Occupational Therapists, Speech and Language Therapists, Nurses and <span class="single_desc_int_gb_word">support staff to deliver coordinated, patient centred care</span>.</p><h4>Package Description</h4><p>You will be joining a <span class="single_desc_int_gb_word">supportive and forward thinking Early <span class="single_desc_int_gb_word">Supported</span> Discharge team who are passionate about stroke rehabilitation and community based care</span>.
