This is a full-time position working 37.5 hours per week.

In this role, you will manage a varied caseload within the Early Supported Discharge service, delivering assessment, treatment and rehabilitation to patients with neurological conditions. You will work both independently and as part of the wider team to provide timely, effective interventions that support safe discharge and recovery at home. You will carry out comprehensive assessments, develop individualised treatment plans and deliver evidence based rehabilitation. You will also work closely with patients, families and carers to set realistic goals and support ongoing recovery. As a Band 6, you will support the wider team through supervision of junior staff, therapy assistants and students. You will contribute to service development and quality improvement, helping to ensure the service continues to evolve and deliver excellent outcomes. Key responsibilities include:

• Assessing, planning and delivering physiotherapy interventions for patients within the ESD service
• Managing a caseload of patients with neurological conditions, including stroke
• Supporting safe and timely discharge from hospital to community settings
• Working collaboratively with multidisciplinary colleagues and external partners
• Providing guidance and supervision to junior staff and students
• Using outcome measures and evidence based practice to inform treatment
• Maintaining accurate and timely clinical documentation

A full UK driving licence and access to a vehicle is essential, as this is a community based role.

Essential

• Degree in Physiotherapy
• HCPC registration
• Membership of the Chartered Society of Physiotherapy
• Post registration experience including work within relevant clinical settings such as neurology, rehabilitation or community services
• Experience managing a clinical caseload and delivering evidence based interventions
• Strong communication, clinical reasoning and organisational skills
• Ability to work independently and as part of a multidisciplinary team

Desirable

• Experience working within stroke rehabilitation or neurological services
• Experience in a community or Early Supported Discharge setting
• Experience supervising students or junior staff
• Interest in service development and quality improvement
