Job summary

<h2>Job Introduction</h2><p>As a Band 6 Physiotherapist, you will join our Early Supported Discharge (ESD) team based at Gravesham Community Hospital in Gravesend. This is a rewarding community role, supporting patients with neurological conditions, including stroke, to transition safely from hospital to home. You will play a key role in delivering high quality rehabilitation within patients own environments, helping them regain independence and confidence. This is a varied role where you can follow the patient journey, making a real and measurable impact on recovery outcomes. You will be part of a friendly and experienced multidisciplinary team, working closely with Occupational Therapists, Speech and Language Therapists, Nurses and support staff to deliver coordinated, patient centred care.</p><h2>Package Description</h2><p>You will be joining a supportive and forward thinking Early Supported Discharge team who are passionate about stroke rehabilitation and community based care. The team encourages learning, development and collaboration, providing an excellent environment for Band 6 therapists to build confidence and progress their careers.
