Main duties of the job

As a Band 6 Occupational Therapist, you will join our Early Supported Discharge Service at Gravesham Community Hospital. This opportunity is designed to support patients recovering from stroke and neurological conditions as they transition from hospital to home. You will be instrumental in delivering patient-centered rehabilitation, helping individuals regain independence in their daily lives. This role is varied, combining inpatient work with community visits, allowing you to follow patients through their recovery. You will work within a dynamic multidisciplinary team that values delivering excellent care and continually improves services. This full-time role involves working across both inpatient and community settings, requiring a full UK driving license and a vehicle for community visits. You will contribute to service improvement, support junior colleagues, and play an active role in enhancing the Occupational Therapy provision.

This is a full time role working Monday to Friday, 8am to 4pm.

In this role, you will deliver specialist Occupational Therapy assessment and intervention for patients with stroke and a range of neurological conditions. You will support early discharge from hospital, ensuring patients receive the right care at the right time in the most appropriate setting. You will work across both the inpatient setting on Sapphire Ward and within patients homes, supporting rehabilitation in real life environments. This approach allows you to make a meaningful impact on patient outcomes, helping individuals regain independence as quickly and safely as possible. Alongside your clinical work, you will support junior colleagues, students and therapy assistants, contributing to the development of the wider team. You will also play an active role in service improvement, working with senior therapists to enhance and evolve the Occupational Therapy provision. Key responsibilities include:

• Delivering specialist, neuro focused Occupational Therapy assessment and intervention
• Supporting early supported discharge for stroke and neurological patients
• Working across inpatient and community settings
• Supervising and supporting junior staff, apprentices and students
• Contributing to service development and quality improvement initiatives
• Working collaboratively with the multidisciplinary team to deliver integrated care

A full UK driving license and access to a vehicle is essential, as you will be completing community visits.

Essential

• Degree or Diploma in Occupational Therapy
• HCPC registration
• Membership of the Royal College of Occupational Therapists
• Post graduate experience across a range of clinical settings, including neurology and rehabilitation
• Knowledge of evidence based practice and relevant national stroke guidelines
• Experience using a range of assessment tools and therapy interventions
• Strong communication, organizational and team working skills
• Confidence using IT systems, including Microsoft Office

Desirable

• Experience working within a stroke or community neuro rehabilitation service
• Experience supervising junior staff or students
• Experience contributing to service development or quality improvement
• Interest in developing specialist skills within neurological rehabilitation
