Job overview

The post-holder will play a key role in ensuring that the Trust provides high quality, patient focused care within the specialist endocrine nursing service; providing expert advice and support to patients, families and health care professionals using best evidence-based practice.

The post holder will work independently/autonomously and at times without direct supervision and will be accountable for their own professional actions. The post holder will be expected to demonstrate highly developed and specialist knowledge and skills through innovative practice leading to the development of increasing levels of autonomy. Using specialist nursing practice, the post holder will manage a cohort of patients with specific and defined endocrine conditions referred for specialist intervention, care and support. The post holder will be expected to establish and maintain close relationships with other Endocrinology services in the region and working collaboratively to improve services for patients.

The post-holder will be a core member of Endocrine Clinical Specialist Nursing Team working in all areas of Endocrinology. The post-holder will focus in supporting endocrine consultant clinics, developing, and running various nurse-led clinics, performing dynamic function tests, carrying out training and teaching session for staff, patients and their families as well contribute to the delivery of endocrine research.

Main duties of the job

• Provide an evidence-based specialist Endocrinology and Metabolism nursing service, to a cohort of patients referred to the endocrinology team. Care will reflect current local protocols and national guidelines and will be delivered through face-to-face consultation within the MDT clinics; by telephone or via electronic communication on a day-to-day basis.
• To ensure that all patient referrals are dealt with promptly and efficiently.
• Demonstrate practice skills in the holistic assessment, planning, implementation and evaluation of care for the cohort of patients within the specialist service. This will necessitate high levels of autonomy and advocacy, using analytical and judgmental skills in undertaking nurse-led consultation to ensure the delivery of appropriate care.
• Manage a cohort of patients and prioritise workload to meet patient and service need.
• Undertake clinical training and supervision to achieve appropriate competencies relevant to this specialist service (e.g. endocrine and late effects nurse-led clinics; dynamic function tests; preparation, administration and monitoring of specialist endocrine treatment; health assessment and non-medical prescribing) to demonstrate expertise in specialist Endocrinology and Metabolism nursing roles.
• Provide counselling to patients and families within the level of skills and expertise.
