
Hello! My name is Dawn and Im the health team manager for the Community Mental Health Team for Older People, East, which is based at Baghill Health and Wellbeing centre, Pontefract within a hub based environment shared with a number of community Mental health teams. We have an exciting opportunity for a full time, band 6 CPN to join our multidisciplinary team. We are looking for an enthusiastic, motivated, and friendly individual who shares the same values as the team, who puts service users first and in the centre of their care.
The OPS CMHT is an integrated team, employing both NHS and Council staff members. We work as an MDT made up of nurses, social workers, support workers, psychologists, medics and admin staff. We work collaboratively with service users to provide treatment based on the service users needs.
We are a friendly, open, and supportive team, valuing supervision and development of all staff.
The service hours are 8.30 am 16:30 pm
The successful candidate will manage a caseload of service users, with a range of diagnoses and needs. They will be responsible for arranging care, liaison with other services and managing and reviewing care plans and risk assessments, ensuring these are done collaboratively and in a recovery focused manner. The successful candidate will also be able to offer evidence based clinical decisions and treatment options. You must be able to work autonomously and as part of a team.
The successful candidate will have a good knowledge of psychotropic medications, medication management and be confident in the administration of depot injections. They will be able to offer support and advise to other disciplines within the team and be confident in their decision making. They will also support in the learning and development of students taking on the practice supervisor and assessor roles for students.
