Joining us as a B6 CPN you will be part of a large multi-disciplinary service 80+ staff supporting transition of patient care from secure settings into the community, which feeds into various sites including Reaside, Ardenleigh, Tamarind, St Andrews and Hillis Lodge (Low Secure) and Elliot House (Approved Premises). We offer extensive support to staff in their role as CC which includes supervision, reflective practice, art therapy, autism support and case-busting sessions. We also provide flexible working patterns which includes working from home when needed, pool cars are also available for community visits. FIRST is a standalone service providing our own home treatment and crisis community support to our service users; our CPN’s and SW’s are on a rota to jointly provide cover for on call shifts (approx. 4 times per month, standby on call payments and overtime are payable).

The current vacancy is for a part time role working 30 hours a week.

Main duties of the job

To undertake the duties of care coordinator in the assessment, planning, implementation and evaluation of the care and treatment needs for a defined group of services users accessing Forensic Intensive Recovery Support Team (FIRST). To provide professional advice and support to other stakeholders involved in service users care which may include families, carers and care agencies, accommodation and housing providers, probation and criminal justice colleagues. To undertake a multi-disciplinary approach to care and working collaboratively with members of the team which may include Responsible Clinician, Social Worker, Occupational Therapist, Psychologist, Support Time And Recovery Worker (STAR), Specialist Substance Misuse Workers, and Peer Mentors. To support colleagues when required with day-to-day tasks and act as a role model and resource to Band 5 CPN’s in the team and student nurses. To advocate for the client group we work with whilst promoting a culture of recovery and optimism.
