We are seeking an enthusiastic and motivated Band 6 Community Children's Nurse to join our dedicated Community Children's Nursing Team. This is an exciting opportunity to work within a highly skilled multidisciplinary service supporting children and young people with complex health needs, long-term conditions, disabilities, palliative care and end of life care requirements across Birmingham.

Main duties of the job

The Band 6 Community Children’s Nurse will provide high quality, evidence based care to children and young people with acute, complex and long term health needs in their homes and community settings. The post holder will undertake holistic assessments, develop and evaluate care plans, and ensure safe, effective and family centred care.

Managing a delegated caseload, the post holder will work collaboratively with children, families, schools, GPs, hospital teams and partner agencies to coordinate care, support hospital avoidance and facilitate timely discharge from acute services.

The role includes supporting children and young people with complex healthcare needs, life limiting conditions and palliative care requirements, helping families to manage care safely within the community. The post holder will contribute to the delivery of specialist palliative and end of life care, including participation in the out of hours on-call service.

As a senior member of the team, the post holder will provide clinical leadership, support colleagues and students, contribute to service development and quality improvement, and maintain high standards of safeguarding, clinical governance and record keeping. They will also provide education and support to families and carers, promoting independence and improving outcomes for children and young people across Birmingham.

Detailed job description and main responsibilities

• Work in accordance with NMC Code of Professional Practice and Scope of Professional Practice. Professionally and legally accountable and responsible for all activities.
• Provide comprehensive evidenced based and skilled nursing care to children within their own home and/or other community settings.
• Ensure assessment, planning and delivery of holistic care is of the highest standard utilising an agreed nursing model.
• Work in partnership with children, families and carers to provide optimum individual outcome acknowledging children and family’s personal preferences and lifestyles.
• Liaise and work in collaboration with acute services, primary healthcare team and other agencies when planning and implementing programmes of care.
• Provide the clinical lead in care packages funded by the Community Complex Care team when allocated to the Community Children’s Nursing (CCN) caseload.
• Provide health education information to the child/young person, family and carers to promote a healthy lifestyle through health promotion activities as required.
• Plan and participate in teaching packages to families and carers to ensure evidenced based care is delivered.
• Advise the Team Leader, Clinical Lead or Service Manager of any circumstances which give cause for concern in relation to patient care and/or any concerns raised by the primary healthcare team or partner agencies.
• Undertake home visits to children registered with GP’s within Birmingham as required.
• Undertake home visit risk assessments to ensure safety during visits especially those during unsociable hours.
• Obtain informed consent from Children, Parents and Carers prior to undertaking care.
• Maintain clear, concise and contemporaneous records adhering to BCHC record keeping policies.
• To be professionally and legally accountable for nursing practice and all activities.
• Provide a flexible service to accommodate the working environment.
