The Transfer of Care Hub is a newly developed team with in BSW who are responsible for covering the 3 identified work streams of patient flow: Front door referrals, Clinically ready for discharge and Out of area placements.
This is a rare opportunity to work within the team as a Bed Capacity Co-ordinator for a 12 month secondment period. Your role will involve the logistical co-ordination of inpatient beds across BSW with the guidance and support of senior colleagues within the Transfer of Care Hub.
Please note this role adapts a hybrid work model, you will at times be required to attend one of our BSW sites in person for whole team working days and team meetings.

Main duties of the job
Support the effective and efficient use of inpatient beds.
Liaise with clinicians across BSW as directed, including external stakeholders to ensure the optimum use of inpatient beds.
To monitor the flow of all inpatient admissions, including liaising with other providers for timely return of any out-of-area placements to minimise financial impact on the organisation.
To contribute to the development of new ways of working and a changing commissioning landscape.

- Build effective working relationships with clinical teams across BSW.
- Develop and maintain effective relationships with others system partner who are involved transfer of care.
- Work as a member of the wider Transfer of Care Hub across the organisation.
- Liaise with members of the multidisciplinary team and other agencies involved in the care of individual service users. Signposting staff, to other services as appropriate.
- Having responsibility for managing your own workload, helping to identify care pathways as appropriate based on best available practice.
- Initiate bed capacity escalations to senior practitioners & service managers.
- To monitor admission dates and intended discharge dates.
- Keep in close communication with all managers re management of the acute care pathway across BSW.
- Work to support the trust to reduce overall out of area and private bed use.
10.Lliaison with colleagues in AWP intensive team to ensure appropriate admissions, ensuring all admissions are gate-kept.
- On a daily basis co-ordinate admissions and transfers to acute wards, Psychiatric Intensive Care Units and ensure appropriate transfers between wards and community services.
- To use word-processing, spread sheet, presentation and graphics handling skills to collate and prepare reports / documents regarding bed use, bed capacity and delayed transfers of care for meetings and governance, including commissioners.
- To confidently manage telephone calls, including the appropriate management of conflict within the system.
- Link with the TOCHub Leadership Team to innovatively develop the acute care pathway, identifying issues and solutions for strategic planning.
- Relay key clinical information, including risks to other stakeholders to support timely access to beds.
- Maintain accurate and up to date records, ensuring that your practice & record keeping complies with all relevant standards and Good Practice Guidelines.
- Contribute to integrated governance processes, including the continuing development, improvement and monitoring of service provision and performance, and monitoring standards of care, through data collection and audit processes.
- Be fully aware of changing government / NHS and Trust policies and the impact these may have on the project.
- Provide support, advice and training as appropriate to other services and agencies involved in the provision of services impacted on by the project.
- Participate in your own supervision and appraisal working with your line manager to set and review your objectives, and to identify and meet your training and professional development needs.
- Participate in providing support, advice and training to staff of other services as appropriate.
- Participate in the development of Trust policies and training programmes.
- Ensure that you keep up to date with developments in care relevant to the service, ensuring that your practice is based on best available evidence and meets all relevant standards and requirements.
- Attend clinical and team business meetings as necessary to achieve the outcomes of the project.
- Manage own workload in order to meet defined objectives guided by general principles, policies and regulations. Be flexible with the ability to alter activities and priorities as the need arises.
- Be aware of the Trust finance structure and the need to make best use of financial resources available.
- To partake in induction of new staff to ensure they are fully informed of the TOCHub objectives.
- To maintain and develop own skills.
- To participate in a review of own performance.
- To chair meetings at a divisional level when required, to keep accurate minutes of meetings when required.
