Job overview

The post holder will work as part of a multidisciplinary team to develop and deliver physical activity and healthy lifestyle management programmes for service users aged 18-25 under the care of the Solihull CMHT HUB. The CMHT HUB provides a service for those aged 18-65 who have complex mental and physical health needs. The post holder will be embedding the principles of individual recovery, focussing on the whole person and building on people’s strengths and experiences. The Post holder will work with the service to work not only on a recovery approach but also on a preventative approach.

Main duties of the job

To promote, maintain or improve health and wellbeing by assisting individuals and communities to adopt healthy behaviours. This can involve such interventions as lifestyle assessments, setting goals for improvement which will support appropriate behaviour/life style changes. The post holder will facilitate both individual work and group work. To work alongside the care coordinators and other disciplines for a designated caseload of service users with complex health needs. To evaluate interventions in order to maintain high levels of care and outcomes for service users. The post holder will work autonomously and in collaboration with the care coordinators to screen and treat service users where their case management is predictable. The post holder will have a working knowledge of local and national policies and legislation which govern current service provision. The post holder will also ensure effective communication pathways exist, acting as a role model to junior staff, students and volunteers within their scope and remit and in a manner that positively reflects the values of the organisation. The post holder will also support the wider MDT as required around service user care.
