An exciting opportunity has arisen for Band 4 Assistant Therapy Practitioners to join our developing Community Admission Avoidance Therapy Team (CAATT). This post will be based at the Royal Gwent Hospital, working Monday- Friday.
The post holder will contribute to the provision of CAATT by providing high quality, effective support to people who have presented to the emergency care department/acute medical units. This activity supports the ABUHB Clinical Futures work aimed at preventing unnecessary admissions to hospital, reducing length of hospital stay, providing care closer to home and establishing home-based support where appropriate by liaising with primary, community and third sector organisations.
We have a post available based at the Grange University Hospital, where CAATT provide a 7-day service 07:30-20:00. The shift pattern can be discussed/negotiated at interview, however, the requirement is for 0.8WTE aligned to these hours of service provision. Whilst the successful candidate will be assigned to GUH as a main base, there will be an expectation that they work across the sites where we provide service when required, with notice being provided, to ensure that the service remains responsive to demands.
Main duties of the job
This role is recognised as a key part of the multi professional service delivery and we are looking for skilled, organised and enthusiastic individuals to join us.
Patient management will be delegated by registered therapists and will include: working with patients to understand their challenges and concerns, the delivery of therapeutic interventions and liaising with patients, family, members of the MDT and external agencies to support and facilitating a return home where possible. Post holders will need to demonstrate good time management, prioritisation skills and flexibility in their approach as well as the ability to communicate effectively both verbally and in writing.
Successful candidates will work alongside registered Occupational Therapists and Physiotherapists and will manage a delegated caseload of patients. The role will include supporting people with a range of functional difficulties within the bounds of the practitioner role, including those presenting with traumatic injuries, multiple co-morbidities, frailty and cognitive deficits.

Assistant therapy practitioners in CAATT are provided with the opportunity to develop and maintain the skills required to meet the defined competencies of the role which includes skills in Dietetic, Occupational Therapy and Physiotherapy support.

- GCSE English & Maths grade C or above (or equivalent)
- Experience of working in a support worker role
- Experience of working in the NHS

- Ability to travel between sites in a timely manner
