Job summary

We are seeking an enthusiastic, hardworking and motivated Assistant Practitioner to join our Lymington Stroke Therapy Team. You will support the multidisciplinary team to deliver a high quality and efficient service to stroke patients across the New Forest.

The role involves completing specified assessments and treatment programmes, working with patients in the community on a one to one or group basis to support rehabilitation exercises and functional activities, and participating in routine Stroke Reviews. The post has a nursing focus and involves close working with the CST Nurse, as well as Occupational Therapists, Physiotherapists, Speech and Language Therapists and other team members.

Applicants should have experience as a carer or therapy assistant, with some knowledge of neurology relating to stroke. The successful candidate will complete stroke specific competencies and receive relevant training, while participating in supervision, service development, the IST programme and a weekend rota supporting our 7day service.

A full driving licence and access to a vehicle for work are essential.

Main duties of the job

To be responsible for the holistic assessment of patients new to the service, as well as on-going assessment of service users on the caseload. It includes the consideration of the clinical, social and psychological needs of both the service user and their carers.

To undertake specified assessments of patients, to interpret assessment

information and plan patient care using judgemental skills to decide upon and

recommend best course of action within competencies / with supervision.

To formulate an individual management and/or treatment programme, using

clinical reasoning skills and a range of treatment skill within competencies.

To progress treatment programmes with specified limits as agreed with senior staff.

To perform and record clinical observations such as blood pressure and pulse. To have a good understanding of when and how to escalate concerns regarding abnormal readings.

To have a basic understanding of common post stroke medications in order to be able to educate patients & their families/carers.

To have a basic understanding of common post stroke nursing needs e.g continence, blood pressure, diabetes and to be able to refer on to relevant services.
