Job summary

The post holder will be a NVQ 3 Qualification/ Associate Qualification and will be responsible for patients allocated to them and will review, manage, plan and deliver care, including being professionally and legally accountable.

Main duties of the job

Be responsible for a delegated caseload following initial assessment by the registered staff, carrying out interventions, including goal setting, outcomes, risk assessment & discharge. To have organisational knowledge relating to Trust protocols and procedures and adhere to them, particularly administration of medicine and moving and handling. To be responsible for providing accurate records of information required by the Trust for audit purposes. To ensure effective risk management at team level by accident/incident reporting, assessing and controlling risk and ensuring residual risks are added to the Trust register. To supervise pre-registration students, Band 3 and 2 and unregistered staff. To employ professional curiosity in your day-to-day role and act upon any findings appropriately. Contribute to the team/service development with the assistance of clinicians and Team Lead Participate in team meetings to ensure effective exchange of information within the multidisciplinary team for optimum care/goal planning and coordination of care delivery Build relationships to promote seamless care

An exciting opportunity has arisen with the Integrated Care Team (ICT North) in Chingford, Waltham Forest, for a compassionate and skilled individual to join our dedicated team as an Assistant Practitioner in Nursing. The role is based at Hawkwell Court and focuses on delivering high-quality nursing care to adults in community settings, including patients’ homes, clinics, and residential care facilities.

As an Assistant Practitioner in Nursing, you will play a key role in supporting the delivery of person-centred care to adults in community settings. Working closely with caseload managers and a wider multidisciplinary team, you will contribute to enhancing patients’ health and independence while supporting the management of long-term conditions and rehabilitation programmes. Your role will be vital in promoting patient wellbeing and ensuring the highest standards of care.

• Deliver direct patient care, including clinical observations, wound care, and delegated nursing interventions.
• Support the management of long-term conditions and rehabilitation programmes tailored to individual needs.
• Promote patient independence, wellbeing, and self-management through education and support.
• Monitor and report changes in patients’ conditions to the appropriate healthcare professionals.
• Maintain accurate and up-to-date patient records using electronic systems.
• Work collaboratively with nurses, therapists, GPs, and wider multidisciplinary teams.
• Contribute to admission avoidance strategies and facilitate timely hospital discharges.
• Provide compassionate care that upholds patient dignity, choice, and respect.
• Proven ability to deliver direct patient care, including clinical observations, wound care, and delegated nursing interventions.
• Experience supporting the management of long-term conditions and rehabilitation programmes.
• Demonstrated ability to promote patient independence, wellbeing, and self-management.
• Competence in monitoring and reporting changes in patients’ conditions.
• Proficiency in maintaining accurate and detailed patient records using electronic systems.
• Strong collaborative skills, with experience working in multidisciplinary teams.
• Understanding of strategies to support admission avoidance and timely hospital discharge.

Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Essex.

Probationary Period This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement – Outer London (B&D, Redbridge, Havering and Waltham Forest, CEME) This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,714 to a maximum of £5,941 p.a. pro rata for part time).
