Job summary

An exciting opportunity has arisen for a motivated and compassionate Dietetic Assistant Practitioner (DAP) to join our team. You will work alongside our current DAP to support our friendly team of Dietitians with patients requiring nutritional support in our community hospitals and those tube fed in their own home.

Main duties of the job

The main duties of this role may include:

Assist registered Dietitians with nutritional assessments of patients

Support the delivery of Dietetic care plans, including monitoring weight, food intake, and hydration status

Provide practical advice to patients and carers on nutrition and food choices under supervision

  • Undertake routine reviews of patients
  • Act as a first point of contact for patients and carers regarding Dietetic queries, escalating complex issues to registered Dietitians Supporting delivery of nutrition training to Trust staff Liaise with nursing staff, medical teams, and community services to ensure continuity of nutritional care Schedule patient appointments, home visits, and follow-ups in coordination with the wider team Support audit and data collection activities to monitor service outcomes and quality improvement Provide general administrative support to the Dietetic team As the role is based at multiple locations across the county, the successful applicant must have access to suitable transport options to meet the travel requirements of the position.

  • GCSE Maths and English (Grade A-C) or equivalent
