Accountable to: AHP Lead, Healthcare in Prison and Professional Lead for Physiotherapy
JOB SUMMARY
The post holder will be responsible for providing a comprehensive respiratory care service in Healthcare in Prison. This includes the day-to-day operational management, triaging referrals, assessment, diagnosis and treatment of patients with respiratory conditions within the HMP Maghaberry, Magilligan and Hydebank Wood College.
The post holder will work closely with AHP Lead, Healthcare in Prison and Trust Physiotherapy Lead and Trust Respiratory Strategic Lead in delivering services, which meet Trust and regional access waiting targets, and in the development and review of services and professional standards.
They will work as an autonomous practitioner using advanced knowledge and clinical reasoning skills to ensure person centred care.
The post holder will provide expert clinical opinion and advice to the Healthcare in Prison team on the management of respiratory patients.
They will also liaise with the Community Respiratory services to ensure appropriate referral and ongoing management for respiratory patients.
Main Responsibilities
Clinical / Professional / Specific Managerial Responsibilities
To be an expert Physiotherapy Advanced Practitioner in all aspects of Respiratory care and practice to patients referred to the service in Healthcare in Prison.
Provide expert clinical advice to other professionals locally and nationally in all aspects of respiratory management across a wide range of acute/chronic conditions, demonstrating advanced skills and knowledge and experience.
To act as a source of clinical expertise in the therapy management of respiratory disease locally and nationally.
To be professionally and legally accountable for all aspects of own clinical and professional work, including patient centred approach to the management of Respiratory care within the Healthcare in Prisons service.
To manage a caseload of very complex cases requiring specialist Respiratory assessment, diagnosis and treatment interventions to ensure intervention is of the highest and evidence-based standards, with consideration given to all treatment options.
To prioritise and plan own workload to manage conflicting and competing priorities both clinically and professionally, to ensure client care and treatment is carried out in an efficient, effective and timely manner in line with Prison and Trust priorities, policies and procedures and statutory legislation.

To undertake comprehensive standardised and non-standardised assessments of clients with a range of respiratory conditions within the Healthcare in Prison service and use advanced clinical reasoning, analytical and communication skills to determine diagnosis and prognosis of condition.
Work with the Community Respiratory Lead and the Trust Respiratory Strategic Lead to develop strategies and protocols within the service to assist with Multi-Disciplinary professional working.
Support the AHP Lead, Healthcare in Prison and Community Respiratory Lead with the implementation of quality initiatives such as HQS Standards.
Together with the AHP Lead, Healthcare in Prison and Physiotherapy Respiratory lead, nursing and medical staff ensure the continued development of staff within the service.
Ensure the provision of an integrated quality service by maintaining effective communication within the Healthcare in Prison multi-disciplinary team and other community services as required.
To integrate and promote evidence based practice within the respiratory service
To create, develop and establish best practice protocols of care and patient care pathways based on evidence based research, clinical governance and local requirements.
To demonstrate and promote best practice through published articles, teaching and attendance at national and international meetings.
To maintain a high standard of clinical documentation and record keeping.
To monitor standards of medical record keeping in all aspects of care for clients within the practitioner led Healthcare in Prison Respiratory service and to address inaccuracies where necessary.
To demonstrate expert skills in assessment and diagnosis of respiratory conditions and to order and undertake appropriate diagnostic testing process.
To independently examine, assess, review and treat patients referred to the Healthcare in Prison Respiratory Clinics. This will include spirometry and non-medical prescribing.
May be required to participate in 7-day working, provision of evening clinics & take part in the Physiotherapy Emergency Respiratory on call service – to be agreed on appointment.
COMMUNICATION
To effectively communicate complex information to patients and carers in order to gain informed consent for and compliance with treatment regimes. This includes situations where there are barriers to understanding and communication in the cases of elderly patient, those with learning disabilities and patients with mental health problems, and where contentious issues, distressing or unwelcome information needs to be imparted.
To effectively communicate with other professions, locally and nationally and other members of the multidisciplinary team to ensure understanding of treatment requirements in simple and complex cases of respiratory disease.
To communicate effectively with Healthcare in Prison clinical staff as well as the medical team within the Hospital Service about any specific patient related problems in order to ensure immediate care and attention.
To communicate effectively with senior members of the Healthcare in Prison team or Respiratory Service if any service related issues affecting patient care.
To communicate effectively and at high level with other specialities within the Trust in the request of specific diagnostic tests.
AUDIT AND RESEARCH
To enhance and promote a culture of research and audit within Healthcare in Prison and the Respiratory Service. Using tools such as audit and outcome based evidence to enhance clinical practice and ensure quality of care.
To participate in audit and research to ensure the safety and relevance of evidence based practice.
To assist in development, monitoring and evaluation of the service provided by Healthcare in Prison and to develop accordingly.
To assimilate and integrate audit and research into the Trust’s R &D policy.
EDUCATION
To promote and maintain an environment which encourages learning and professional development amongst all members of the community respiratory service.
To provide multi-disciplinary training and education within the Trust, locally and nationally.
To participate in the training and education of all disciplines at under and postgraduate level.
To identify the learning development needs of the Respiratory care Team.
Assist in a peer review programme for senior members of the Respiratory care Team.
Disseminate knowledge of clinical practice and audit and research findings at national and international meetings and through journal publications.
To enhance links between professional bodies and other specialities.
MANAGEMENT
Ensure the development of the Healthcare in Prison Respiratory service in line with strategic direction and goals of the overall service.
Develop innovative practice according to service needs and requirements.
To ensure workload and caseload levels are monitored and controlled to ensure maintenance of high standards of care.
Participate as required in the recruitment and selection of staff in accordance to Trust policies.
To provide (be accountable for) effective guidance, supervision, leadership and appraisal to the wider team.
Any other duties as required by the trust.

You will be working with vulnerable adults with mental health needs who are in custody for a wide variety of reasons including violence and sexual offences. You will be required to adhere to all South Eastern Trust, Northern Ireland Prison Services, regional and national policies, procedures and standards.

A minimum of 5 years clinical experience in respiratory services with at least 2 years at senior level (band 6 level or above).
Senior experience must include at least 12 months working in the treatment and management of patients within respiratory care.
Minimum of one MSc. module and at least one other relevant validated post graduate course relating to the management of respiratory patients.
PG Certificate in Medicines Management enabling Registration as an Independent Non-medical prescriber (NMP).
Have advanced skills relevant to diagnosis, assessment and treatment of respiratory patients eg sputum challenges, spirometry
Evidence of specialist /advanced training or willing to undertake to meet service objectives eg. IRMER and interpretation of chest imaging, Blood sampling- ABG, Venepuncture.
