As part of South Tyneside and Sunderland NHS Foundation Trusts continued commitment to improving the safety of patients and the effective utilisation of resources, we have an exciting opportunity for an Advanced Practitioner Pharmacist in the Division of Surgery. You will be required to lead and co-ordinate the professional clinical input into the delivery and development of services in the designated directorates. You will be expected to work closely with Directorate Lead Pharmacists and Advanced Practitioners within the Division in supporting the wider pharmacy team including the delivery of care from pre-assessment clinics, admission for elective and non-elective cases, through the inpatient stay and beyond into primary care.

Main duties of the job

You will be able to provide evidence of effective service delivery at a specialist pharmacist level, post-graduate study to diploma level and well-developed clinical pharmacy skills with an independent prescribing qualification and evidence of advanced clinical skills development. The ability to support and mentor junior pharmacists and undertake clinical supervision will be key skills.

The Pharmacy Service operates a seven-day clinical service with routine support of both junior pharmacists and integrated medicines management technicians.

South Tyneside and Sunderland NHS Foundation Trust have invested in modern pharmaceutical facilities including full electronic prescribing with decision support, automated dispensaries and extensive seven-day clinical services to enhance the patients experience of care by improving the quality, safety and effectiveness of their medicines utilisation. As part of this investment our nationally recognised pharmacist-led Integrated Medicines Management Scheme has been expanded to all acute areas. Your role will be to lead and co-ordinate the professional clinical and pharmaceutical input for this patient group with a focus on medicines optimisation, safe and secure utilisation of medicines and minimising the risk of inappropriate use.

Job responsibilities

PLEASE REFER TO THE ATTACHED JOB DESCRIPTION FOR FULL DUTIES OF THE ROLE.

• To lead and co-ordinate the professional clinical and pharmaceutical input into the safe and cost effective utilisation of medicines in conjunction with the Trust surgical care services and lead consultants.
• To provide expert advice in the designated area undertaking risk management, financial management and clinical governance activities and undertake prescribing to deliver medicines optimisation and safe and effective discharge.
• To monitor performance of pharmacists working in this area, to undertake clinical supervision and internal and external teaching for all staff.
• To co-ordinate clinical service delivery in conjunction with other advanced practitioners and directorate lead pharmacists to ensure safe and efficient care from pre-assessment to discharge.
• To link with the antibiotic lead pharmacist to deliver effective antibiotic utilisation and initiation to reduce the incidence of Hospital Acquired Infections.
• To work at an advanced level in the designated areas of practice.
