We are seeking a highly skilled and motivated Band 8a Pharmacist to join our care homes team at our Halesfield premises in Telford. This is a part-time position, working 15 hours per week, on a fixed-term contract of 6 months. As a Pharmacist, you will provide expert pharmacy support to our care homes, enriching our multidisciplinary team (MDT) and ensuring the safe and effective use of medications. This role is crucial in supporting the deprescribing of medicines and optimising therapy to keep residents at home and prevent hospital admissions. It aligns perfectly with the new NHS 10-year plan, which focuses on moving treatment to prevention and hospital to community care.

Main duties of the job

  • The post holder will act as an expert clinician within the care home environment, to ensure patients can access care closer to home whilst supporting the delivery of the Trusts strategic objectives. Responsible for the provision and development of clinical pharmacy services in care homes across Shropshire, Telford and Wrekin (STW).
  • Be a key member of the multidisciplinary team in relation to care homes.
  • Support development of the care home pharmacy service to ensure patients discharged from hospital are assessed and medically optimised, including their pharmaceutical needs to reduce the risk of readmission.
  • Support care homes in conjunction with the ICB in educating and training staff on the safe storage and use of medicines.
