We are looking for a dynamic, experienced and highly skilled ACP to join our Merton Home Treatment Team (HTT) in a permanent 0.5 WTE 18.75hrs Band 8a position, based at Wilson Hospital.
This is an exciting opportunity to work as a senior autonomous clinician within an experienced multidisciplinary Home Treatment Team, supporting people experiencing acute mental health crisis to receive high-quality treatment and care within their own homes wherever possible.
You will be a registered healthcare professional with an MSc or PGDip in Advanced Clinical Practice, an independent prescribing qualification, current professional registration and experience working at an advanced level of practice. You will bring strong clinical assessment and risk-management skills, confidence in managing complexity and the ability to work autonomously while building effective relationships across the multidisciplinary team.
Main duties of the job
The successful candidate will be predominantly based within Merton HTT, with one day per fortnight working within the Crisis Access Hub. The Trust's ACP role encompasses advanced assessment, formulation, diagnosis, treatment planning and non-medical prescribing, alongside clinical leadership, education, research and quality improvement.
Detailed job description and main responsibilities
Within Merton HTT, you will provide advanced clinical expertise for people experiencing acute and complex mental health difficulties. You will work autonomously while remaining an integral part of the multidisciplinary team, supporting clinical decision-making, treatment and discharge planning.
You will also spend one day per fortnight within the Crisis Access Hub, giving you the opportunity to contribute to advanced clinical screening, enhanced triage, risk assessment and pathway decision-making at the point people access crisis services. The Crisis Access Hub provides a single point of access for crisis referrals across Kingston, Merton, Richmond, Sutton and Wandsworth.
Main duties will include
- Delivering expert autonomous mental health assessment, formulation, diagnosis and treatment planning.
- Providing non-medical prescribing within your professional scope of practice.
- Providing senior clinical leadership and supporting decision-making for complex presentations.
- Working collaboratively with service users, carers and the wider multidisciplinary team.
- Supporting colleagues through clinical supervision, coaching, education and role modelling.
- Contributing to safe and effective treatment and discharge planning.
- Supporting clinical governance, audit, service development and quality improvement.
- Contributing to the development of future ACPs and the wider clinical workforce.
Use of Artificial Intelligence (AI) in job applications
If you have used AI to support the preparation of your application, please briefly indicate how it was used within your supporting statement or application form.
Please remember, your application is your opportunity to tell us about your skills, experience and motivation for the role. We encourage applicants to complete their application and supporting statement in their own words wherever possible. We recognise that AI tools can be helpful in supporting accessibility, and for improving spelling, grammar and structure.
Applications that rely heavily on AI-generated content can become generic and fail to accurately reflect an applicant's individual experience. Applications that clearly demonstrate your own voice, reflections and experiences are often the most effective and help us better understand your suitability for the role.
Please be aware that information provided in your application and supporting statement may be explored further during the selection process. It is important that all applications accurately represent your own knowledge, skills and experience.
