An exciting opportunity has arisen for an advanced clinical practitioner to join the evolving Acute Frailty Team on a fixed term basis. The Acute Frailty Team currently provide an in-reach service into the Emergency Departments (ED) at both our St Mary’s and Charing Cross hospitals, assessing patients using a comprehensive geriatric assessment (CGA) framework, with the aim of admission avoidance.
The successful applicant will have experience working with frail patients, ideally within specific frailty services. The role will involve specialist assessments to expertly meet the needs of this complex cohort of patients. Most importantly, the successful applicant will have a passion for older people’s care.
Please note that this position requires the successful applicant to be able to cover both the St Mary’s and Charing Cross sites. Currently the service does not operate at weekends and bank holidays. This is subject to change as the service expands.
Main duties of the job
- Using referral criteria, clinical knowledge and experience, identify and initially assess patients within ED and assessment areas that are suitable for a CGA.
- Independently assess, diagnose and manage patients presenting with acute and chronic conditions as well as non-specific symptoms, identifying the underlying causes.
- Request, interpret and act on diagnostic investigations and imaging as appropriate.
- To contribute to the development of programmes of education to enhance the nursing contribution and quality of care to patients, to be delivered both locally, regionally and nationally.
- To contribute to the development of nursing & AHP practice within the team. Additionally, where appropriate, to act as a practice supervisor to Trust and honorary contract employees.
- To participate and assist with any Trust approved research projects conducted within the department including identification of suitable patients.
- To have an excellent working knowledge of key Trust targets and prioritises in terms of operational/strategic plan for the service and related initiatives; to actively support the Trust in delivering high quality care and maintaining Care Quality Commission Compliance.
