This is a 12 month secondment/Fixed term post to cover maternity.

Chesterfield Royal Hospital is looking to appoint a fully qualified ACP to join our established, innovative, and improvement-focussed acute frailty team  on a 12 secondment or fixed term contract.

The overriding purpose is to provide early Comprehensive Geriatric Assessment to those who need it most. The successful applicant will use their advanced clinical skills to support the provision of the highest quality patient care through personal actions and aiming for continuous improvement.

Core components of the role will be ability to deliver education, provide leadership and become involved in service development once established in the post.

Main duties of the job

To provide high quality clinical assessment, intervention, and treatment for a wide variety of older people living with frailty. Early identification of those requiring Comprehensive Geriatric Assessment and initiation of the process by becoming a core member of the Acute Frailty multidisciplinary teams. Using their expertise in frailty to advocate for and support the patient journey.

Working closely with the Acute Frailty consultant Geriatricians.

To work independently at an advanced clinical level in the direct delivery of high-quality care making critical clinical decisions based on advanced clinical skills assessment.

The post holder will demonstrate safe clinical decision making and expert care including assessment and diagnostic skills for patients.

The post holder will provide clinical leadership and support to improve the patient experience and provide safe, quality care.

To support junior medical staff sharing clinical workload and allow education and training requirements to be met.

The post holder will work collaboratively with clinical teams to meet the needs of patients, supporting the delivery and development of policy and procedures and providing appropriate leadership as required ensuring the maintenance of clinical excellence.

Experience

Essential

  • Substantial broad experience relevant to the care of frail older people
  • Previous experience at managing/leading a team

Desirable

  • Knowledge of and experience in General Surgery/ Orthopaedics/ Urology/ Critical Care
  • Experience of working within a Frailty team
  • Project management

Qualifications

Essential

  • Registered healthcare practitioner (nurses, physiotherapists, paramedics, pharmacists)
  • ALS provider (or willing to complete)
  • Already undertaking & due to complete their MSc in Advanced Practice within a timeframe of 3-5 years

Desirable

  • Completed Masters degree in “Advanced clinical practice”
  • ALS instructor
  • Independent prescriber
  • Ability to perform basic interpretation of CT brain & Skeletal X rays

Skills & Knowledge

Essential

  • Broad & advanced knowledge of clinical pathophysiology
  • Has a sound level of knowledge in relation to invasive and non-invasive diagnostic and therapeutic procedures
  • Excellent communication skills with clear written and oral skills.
  • Clinical audit experience
  • Evidence of proven leadership ability
  • Willingness to perform and or learn advanced clinical skills appropriate to the area of practice
  • Ability to manage a team of patients and prioritise workload

Desirable

  • Management skills- effective organisational abilities, supervisory skills, leadership skills, change management skills. Clinical governance, current health service priorities. National and local nursing agenda
