Cornwall Partnership NHS Foundation Trust is shaping a new vision for Community and Palliative End of Life Care across the county. Our goal is to support people nearing the end of life to have a calm, peaceful, and dignified death in the place of their choice.
Cornwall has one of the highest rates in the country for hospital stays during the last 90 days of life. This highlights the need for a patient-centred, community-based service that allows more people to receive the care they need at home.
We’re building a service that reduces unnecessary hospital admissions, offers a single point of access, and delivers personalised care plans for home-based support.
In partnership with the Macmillan Social Investment Fund and local organisations, we’re expanding services, including a 24/7 Rapid Community Response and Care Service, and a dedicated phone line for both the public and professionals focused on End of Life Care.
These developments will complement our existing Hospital@Home virtual service and new partnerships with the voluntary sector to enhance local care.
