Job summary
The Advanced Clinical Practitioner will make autonomous decisions in the assessment, diagnosis and treatment of patients registered with our practices. You will have experience within a primary care setting and your high standards of patient care will naturally inspire those you work with.
Main duties of the job
As a clinical leader within our multidisciplinary team (working alongside GPs, Practice Nurses, Care Coordinators and Pharmacists), you will help transform and modernise care pathways.
Your day-to-day will involve:
- Autonomous Clinical Practice: Making independent decisions in the assessment, diagnosis, treatment, and management of patients across full episodes of care.
- Minor illness & long-term conditions.
- Clinical Governance & Leadership: Accountable alongside the Lead Nurse for driving clinical governance, ensuring safe, compliant, and high-quality service delivery.
- Team Mentorship: Inspiring high standards of care, supporting skill-sharing across traditional professional boundaries, and mentoring team members.
