We are excited to offer an exciting opportunity for an Advanced Clinical Pharmacist – Palliative Care to join our expanding team at University Hospitals Liverpool (Aintree Site). This is a particularly exciting time to join us, as our palliative care pharmacy service is growing and leading several innovative projects across both primary and secondary care including virtual wards. This role provides a fantastic opportunity to develop specialist expertise in palliative care while playing a key role in service development and quality improvement.

Our vision is to deliver a Trust-wide palliative care pharmacy service, working collaboratively across directorates to ensure the safe, effective, and evidence-based use of medicines in patients with eye conditions. Based at the Aintree site, the successful candidate will work closely with our consultants, specialist nurses, and multidisciplinary teams, gaining hands-on experience in managing complex palliative patients in both inpatient and virtual ward settings.

Main duties of the job

Assist the Consultant Pharmacist- Palliative Care and Lead Divisional Pharmacist for Medicine (Aintree site) in the development of pharmacy services and medicines management within the Palliative Care service.

To provide a safe and effective advanced clinical pharmacy service to the Palliative Care service (inpatients and virtual wards)

Assist the Lead Divisional Pharmacist for Medicine (Aintree site) in providing clinical training and supervision, relevant to the Palliative Care service, for other departmental staff.

Provide outpatient clinic patient review and support where applicable to your role.

Provide Advanced Clinical Pharmacist support to palliative care virtual wards

Provide non-medical prescribing services to the Palliative Care service.

To work with the current Consultants Pharmacist in the Palliative Care service in delivering the key responsibilities.

With support from the Lead Divisional and Clinical Pharmacists audit, monitor and develop medicines management within your speciality.

Detailed job description and main responsibilities

Assist in the delivery of the Pharmacy Strategy, contributing to service development and innovation.

Lead on the development, implementation, management, and audit of medicines policies and clinical guidelines across the Trust, ensuring safe and effective prescribing for patients with acute and chronic eye conditions.

Supervise and support the training of pharmacy staff in pharmacotherapy, including prescribing considerations for conditions for patients requiring palliative care

Liaise with healthcare professionals across primary and secondary care to ensure efficient use of  medicines and resources, and where appropriate, develop shared care protocols for managing long-term eye conditions.

Collaborate with  consultants, multidisciplinary teams including specialist nurses to optimise pharmaceutical care and improve patient outcomes.

Support specialist clinics including virtual wards, ensuring appropriate prescribing, monitoring, and patient education for therapies.

  • Vocational masters degree in pharmacy
  • Membership of The General Pharmaceutical Council
  • Diploma in clinical pharmacy or equivalent documented clinical experience
  • Full NMP qualification
