Midlothian is growing clinical capacity across Primary Care in fulfillment of the delivery of a pharmacotherapy service for General Practice.
An exciting opportunity has arisen to join our award-winning primary care team, which is fully engaged in the delivery of a range of primary care, hospital and pharmacotherapy activities. The successful post holder will work in general practices delivering services locally as well as centrally with our team in the medicine’s reconciliation hub. 50% of the post has commitment to the Midlothian Community Hospital, allowing the successful candidate to work with our hugely respected pharmacy team delivering services to inpatients.
We require experienced pharmacists to deliver a range of clinical pharmacy services including polypharmacy reviews and assessment of patient medicines, with services delivered to patients living at home and in care homes around the county as well as hospital inpatients. You will be responsible for monitoring service delivery and providing reports to key stakeholders including Health and Social Care Partnership clinical leads. The Midlothian Pharmacy service is a supportive and nurturing team fostering an environment that allows individuals to flourish. We promote leadership at all levels.
The Primary Care Pharmacy Service is enthusiastic about participating in practice-based research using quality improvement methodology
