Job overview

  • The PHNA will work under the direction of Specialist Community Public Health Nurses (SCPHN) and Registered Nurses (RN) assisting them in promoting the health and development of school aged children resident or attending schools  in the area. This includes participating in the planning, implementation and evaluation of programmes of care to meet the specific needs of identified children and their families; working in a variety of settings with individuals, groups and the wider community.

They will connect children and families to resources within the borough that will promote early intervention, build resilience within children, young people and families.

The post holder will be instrumental in delivering different elements of the Healthy Child programme including the National Child Measurement Programme, health promotion programmes in response to delegated work identified by the Specialist Community Public Health Nurses (SCPHN)

The post holder will work with specialist SCPHN’s in delivery specific delegated packages of care a range of service providers to identify children and families at an early stage who will benefit from follow up and support to avoid unnecessary use of unscheduled care, to make use of local resources and to achieve good outcomes for children.

Main duties of the job

  • To provide clinical support such as health promotion activities, one on ones or group work to the in-a variety of settings for example, The home, Schools, Health Centres, Family Hubs & Youth centres.
  • With support from, the Specialist Community Public Health Nurses (SCPHN) deliver both group and individual health promotion sessions as part of the commissioned service in a variety of settings. These sessions may be group e.g. class assemblies or on an individual basis.
  • Under the supervision of Specialist Community Public Health Nurses (SCPHN)/ registered nurses, provide evidence-based 1-1 advice for children and young people on issues such as continence, healthy eating, oral health immunisations, and emotional well-being promotion of their health at set stages for children and young people, enabling successful completion. This would be  following a needs-based assessment.
  • This promotion includes assemblies and liaison with school staff to facilitate questionnaire completion.
  • To support in the delivery the National Childhood Measurement Programme for Reception and year 6 in primary schools and the vision screening as part of the PHN 0-19 service and the documentation.
  • To understand child protection issues relating to the safeguarding of vulnerable children and young people and document and report to the School Nurse issues of concern.
