The CAMHS Tier 4 Transitions Team is looking to recruit a Young Person and Family Outreach Worker to work alongside the Practitioners within the team to support young people in planning and actioning their discharge plan from an inpatient setting back into the community or next assessed location. This is a PAN Dorset post and support young people and families from Dorset. The role itself is both demanding and rewarding; a flexible, hardworking approach is essential to managing the demands of working within an inpatient setting whilst also being creative in the community space to support young peoples identified goals.
This post is offered on a Part-time basis covering a secondment opportunity within the team. The successfully applicant will be expected to work a variety of shifts with flexibility in mind. The post holder will need to have experience of working therapeutically with children and young people with mental health presentations and their families. Experience of working in mental health services is essential.
Main duties of the job
To work with Children and Young People on the CAMHS Transitions Team caseload who require a creative, time limited intervention to achieve person centred goals to help young peoples ongoing journey back into the community.
Working as part of the wider multidisciplinary team to provide therapeutic interventions to children and young people with high risk and complex presentations, Supporting their opportunities to engage in community activities such as education, clubs and time with their family and friends.
Ensuring the highest standards of service are provided; to ensure that services to young people and their carers are delivered and developed accordingly to Trust, Social and Healthcare departmental policies.

Must hold a full current UK Driving Licence and business insurance. (will have access to the ward vehicle)
This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
