Tower Hamlets Community Learning Disability Service (CLDS) is seeking a Weight Management Programme Coordinator (AFC Band 4) to work closely with our Dietitian to support the delivery of our Tier 2 weight management programme, including Shape Up.  Shape Up involves working closely with adults with learning disabilities and their support networks to increase physical activity levels and improve diet and eating habits.  Shape Up includes group teaching and 1:1 interventions delivered by the Programme Coordinator over a twelve-week course with several courses running at any one time.  The postholder will also deliver training courses and support to paid and unpaid carers.

This is an exciting opportunity to work collaboratively with adults with learning disabilities, family carers and  support staff (day services, homecare, supported living) and the post is based in a large integrated health and social care team.  The role includes programme delivery, promotion and planning; referral screening and triage; developing resources;  data gathering and reporting.

We are looking for someone with excellent communication, IT and organisational skills, and confident to deliver training, both in person and online.  You will be supervised and supported from within CLDS.  This is a fixed-term post initially (ending 31/3/26).

Main duties of the job

  • Working with adults with learning disabilities, their families and paid carers
  • Delivering training and information about weight management and healthy lifestyles to people with learning disabilities, family carers and paid support workers
  • Processing referrals and recording data (eg. height, weight)
  • Working in a range of community settings and people's homes within Tower Hamlets
  • Networking with organisations who work with adults with learning disabilities

We are looking for someone with experience of working with people with disabilities who has good verbal and non-verbal communication skills and an interest/experience in promoting healthy lifestyles and weight management.   You will also need to have experience of presenting information to groups of people and to have good computer skills (eg. use of Excel, Powerpoint, MS Teams).

Key responsibilities:

  • Promoting the programme to stakeholders eg. clients, provider organisations, GPs
  • Assist in triaging referrals to establish eligibility for the programme
  • Planning and organising programme delivery – identifying participants for each cohort, venues, gathering and developing programme resources, session planning, identifying and supporting volunteer peer coaches
  • Programme delivery – working closely with participants and their support networks to develop and review individualised goals and timetables, delivery of programme content to participants, both in groups and 1:1
  • Data gathering and reporting – measurement and collation of baseline and repeat data for participants, data entry, monitoring data and preparing reports; gathering service user feedback
  • Developing working relationships with clients, provider organisations, GPs, local community resources
  • Support in the delivery of the dietetic service within CLDS.
