We are seeking a Band 6 Practitioner to provide rapid assessments and interventions, helping to keep people safe and supported in their own homes, avoiding unnecessary hospital admissions. This post requires a full drivers license and use of your own car.
As part of the Urgent Community Response service, you’ll work with a multidisciplinary team to deliver high-quality care to patients in crisis, responding within a 2-hour timeframe. This role is fast-paced, rewarding, and essential to delivering integrated, patient-centred care in the community. Our team also supports clinical delivery of the Virtual Wards (Hospital@Home) service, providing clinical management to patients with acute illnesses within the place they call home, rather than in a hospital bed.
Main duties of the job
The post holder will be an experienced Registered Nurse in Adult Nursing or a HCPC registered Paramedic who will work across the UCR services. We would encourage applications from professionals with high level clinical skills, knowledge and experience in a constantly evolving environment.
As a Band 6 urgent community response practitioner you will:
- Provide rapid assessment, intervention, and care to patients experiencing an acute illness or sudden deterioration in their health within the community.
- Collaborate with a multi-disciplinary team to create individualized care plans, ensuring a holistic approach to patient care.
- Act quickly and effectively to stabilize patients and arrange necessary treatments, referrals, or hospital admissions when needed.
- Work closely with GPs, social care, and community services to provide seamless care and maintain patients' independence at home.
- Lead and support junior staff, offering guidance and supervision as needed.
- Take part in the development and implementation of clinical protocols, ensuring the highest standards of care are met.
The UCR team works 7 days a week covering shifts between 8:00 until 22:00. You will be expected to work weekends and bank holidays.
