Main duties of the job

Key Responsibilities:

• Clinically triage referrals to community nursing services, prioritising according to clinical need and risk.
• Provide timely clinical advice, signposting, and safety-netting to patients, carers, and professionals.
• Allocate and coordinate workloads within the community nursing team.
• Escalate concerns appropriately and support urgent or complex cases.
• Maintain accurate and contemporaneous clinical records.
• Work collaboratively within the multidisciplinary team to support seamless patient care.

This role presents an excellent opportunity to be involved at an exciting time, with real scope to help shape how community care is delivered across the City of Leeds, supporting both planned and unplanned care pathways.
