At West London NHS Trust, we are delivering an ambitious multi-year Transformation Plan to reduce inequalities, improve access and strengthen care closer to home.

We are seeking an experienced senior programme who is passionate about neighbourhood-based care. You will lead complex, system-wide programmes that improve access, equity and patient flow whilst enhancing quality and patient experience.

Working with clinicians, service users and system partners, you will shape and deliver integrated, clinically led and financially sustainable transformation as part a supportive and innovative team.  This role suits someone outcomes-focused, collaborative and motivated to create lasting change in mental health and community care.

Main duties of the job

Reporting to the Head of Transformation, this job holder will work across community and mental health programme areas in leading end-to-end delivery of key transformation priorities. The job holder will lead the end-to-end delivery of key transformation priorities, ensuring robust and effective change management throughout the process. This will involve:

  • Responsibility for the day-to-day management of assigned deliverables including starting up, maintaining, supporting, facilitating, monitoring progress, closing and evaluating multiple workstreams.
  • Managing multiple workstreams, from initial planning, development and implementation to ongoing support, monitoring progress, and final evaluation.
  • Providing guidance and expertise to support clinical and operational leaders on delivery of transformation initiatives, including overseeing project planning, resource allocation, risk management, and communication.
  • Responsibility for all enabling work to drive the programme including co-production with service users, carers, staff and co-production partners; enabling digital solutions; enabling estates solutions; outcomes; and workforce.
  • Lead delivery through close working with senior management, clinical leads and support services in leading delivery of programme priorities.  They will also work closely with internal and external colleagues ensuring they are fully sighted and involved in all transformation work.
