An exciting opportunity has arisen for an enthusiastic and forward-thinking Band 6 Nurse to join the Transfer of Care Hub (Integrated Discharge Team) Fixed Term or Secondment until end of March 2027.
The successful candidate will promote the Discharge to Assess model by facilitating safe and timely discharges for patients from Macclesfield District General Hospital, supporting the Acute Medical Ward and and as necessary for patients in Community Pathway 2 beds Transfer to Pathway 3.
The successful candidate will have a passion for putting the patient at the centre and have experience of working collaboratively with multi-professional partners. The job will include supporting the completion of assessments and continuing healthcare checklists within the pathway 2 beds. The post holder will need to be able to travel within Cheshire East to support this.
Hours per week: 37.5 hours

To coordinate and facilitate the safe and effective discharge of patients from Macclesfield District General Hospital supporting proactive reduction in delayed transfer of care.
To provide assessments for discharge pathways 1-3.
Working proactively within an integrated health and social care team, to promote formal and informal channels of communication both within the team, ward staff, ICB and wider community as appropriate.
