THERAPY ASSISTANT PRACTITIONER – IN-PATIENT REHABILITATION - Beechwood at Spinneyfields, Rushden

BAND 4

(37.5 hours week – over 5 days – includes weekend and bank holiday work)

The post holder will be working in an integrated multi professional inpatient team providing quality care for a group of patients, specialising in rehabilitation and sub-acute care from patients stepping down from acute hospitals and step up patients from the community in a physical health inpatient unit.

As the focus of the centres is rehabilitation you will be working in an integrated multi-disciplinary team that includes senior nurses, staff nurses, HCAs, physiotherapists, occupational therapists, therapy assistants, medical staff, discharge coordinator and wider external colleagues.

The services will provide a unique opportunity to work with a dedicated team to deliver vital intermediate care for patients in Northamptonshire.

Main duties of the job

We want to create an effective and high performing team and so we are looking for somebody who is proactive and dedicated team member, is open to new ideas and is committed to delivering high quality patient care. The centre is currently being planned for an expansion to 51 beds including rehabilitation, slower stream rehabilitation, surge beds and supporting patients with resolving delirium and dementia. You can be part of this new service from the very beginning and make a positive difference to people’s recovery.

Examples of specific care that the post holder includes undertaking assessments such as initial therapy assessments, setting therapy goals with patients, plan therapeutic interventions and then evaluating the reassessing needs as necessary with indirection support from Registered Therapists.  The therapeutic interventions will  involve personal care, toileting programmes, dressing assessments and practice, moving and handling, demonstrating use of equipment to patients and carers, initiating therapy group sessions,  basic home assessments with patients, ordering equipment and other activities such as kitchen assessments and practice.  You be expected to communicate with patients who may have communication difficulties, families and other colleagues and documenting care provided on a computerised system.  The post holder will be involved in the induction of junior colleagues and provide support and guidance as necessary.
