Main duties of the job

The Transfer of Care Hub Team Manager is responsible for providing leadership and operational oversight to a multidisciplinary team coordinating safe and timely hospital discharges. Key duties include managing staff performance, rotas, and development; supporting service transformation aligned with national discharge policy; ensuring compliance with safeguarding, clinical governance, and infection prevention standards; and promoting integrated working across health and social care. The role requires strong leadership, communication, and problem-solving skills, alongside knowledge of hospital discharge pathways, the Mental Capacity Act, and system-wide partnership working. The successful candidate will be adaptable, emotionally resilient, and committed to delivering high-quality, patient-centred care through effective team and service management.

Detailed job description and main responsibilities

Provide direct line management to all staff within the Transfer of Care Hub. • To provide leadership support to the designated health and social care team. • To promote a strong, joint team structure, culture and ethos that staff from both health and social care identify with. • Support patient flow within the Transfer of Care Huband intermediate tier services within Warrington.

  • To ensure that Trust and the Local Authority fulfils its legal regulatory and national minimum standards requirement. • To maintain and utilise a professional, evidence informed knowledge base to lead on developing the service accordingly with all relevant stakeholders. • To provide overall management and leadership for the multi-disciplinary team within the Integrated Service and along with integrated team managers take a lead role in their development, in the context of the transformation agenda. • To ensure effective service delivery and development, ensuring delivery of the associated key performance targets/defined outcomes. • To support the Service manager with the development of service initiatives in line with agreed transformation agenda including service re-design. • To manage risks within the designated area and ensure robust safeguarding arrangements. • In conjunction with the Service Manager, ensure the continuing cost effectiveness of the service ensuring it is efficient and effective in accordance with the vision, values and strategies of Warrington Borough Council and Warrington and Halton Hospitals NHS Trust. • To support the Service manager with performance of the services within the designated area and provide reports on performance to relevant boards as require
