Central London Community Healthcare NHS Trust (CLCH) – are looking to recruit a Team Lead - Therapist for an exciting opportunity within the intermediate care team (ICT).

Are you a Band 7 or an experienced Band 6 Occupational Therapist or Physiotherapist looking for an exciting, new challenge and a chance to take on greater responsibilities?

The ICT covers several community pathways involved in supporting hospital flow and supporting patients in the community in Wandsworth. These are:

  • Care transfer hub – facilitating discharge and hospital flow in partnership with the St Georges discharge transfer of care hub. Acting as a liaison/access point for CLCH services
  • Discharge to assess community therapy team – follow up in the community of patients discharged from hospital to ensure smooth transition into community and avoid readmission.
  • Quickstart service - provides rapid response social care services for patients who are at risk of social care breakdown in the community or to help facilitate hospital discharge
  • Phlebotomy – provision of phlebotomy for housebound patients

The B7 will lead one of the primary pathways above, with the expectation to cross-cover across all pathways within the intermediate care team

Staff are expected to utilise CLCH bases across Wandsworth if necessary.

Main duties of the job

The ICT provides a 7 day a week service.  Examples of duties are below;

  • Liaising with the St Georges transfer of care hub, emergency department and ward teams to ensure timely discharge and linkage with CLCH service
  • Community therapy assessment and rehabilitation for patients recently discharged from hospital. These patients are usually more urgent / complex / have higher risk
  • Supporting the Quickstart care service with management of patients referred for urgent care

Risk management, disability management and rehabilitation are key components of the service delivery.

The team sees a wide variety of conditions/pathologies and provides therapy in a holistic way using a wide variety of knowledge and skills.  The team consists of Physiotherapists, Occupational Therapists, rehabilitation assistants / healthcare assistants & phlebotomists in a multidisciplinary team.
