Job overview

An exciting opportunity has arisen for a proactive and dynamic candidate to provide operational leadership and line management to the Community Access Team, who are based at the Norfolk and Norwich University Hospital NHS Foundation Trust.

The Team Lead will manage the workforce, take responsibility for quality monitoring and work collaboratively with system partners.

Our team consists of Senior Nurses, Discharge Co-Ordinators, Senior Physiotherapists, Senior Occupational Therapists and Assistant Practitioners.

At the front door, we provide an Admission Avoidance service which runs from 8AM to 8PM, 7 days a week.

At the back door, we support patient transfers from Norfolk and Waveney acute hospitals to our Intermediate Care beds across Norfolk.

The successful candidate must be able to make own travel arrangements. Please note that there isn't parking available on site at Norfolk and Norwich Hospital.

  • To be responsible for the day-to-day operational line management of the team, leading and ensuring a 7 day 8am-8pm service is provided.
  • To support the team's  Admission Avoidance service.
  • To support the discharge of patients to intermediate care beds working within statutory timeframes and operational targets.
  • To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users

Detailed job description and main responsibilities

  • To be responsible for the day-to-day operational line management of the team.
  • To plan, coordinate and evaluate the services and ensure they are safe, efficient and effective with regards to patient management and use of staff time.
  • To facilitate development of the integrated team and individual staff members to promote job satisfaction, performance and an ability to manage change.
  • To actively perform absence management, ensure absence reporting is undertaken in a timely manner, as per the Trust’s Policy, working closely with HR.
  • To deputise for the Integrated Service Lead as required.
  • To work collaboratively with healthcare professionals and statutory / non-statutory agencies to provide a seamless, integrated service to our service users.
  • To adapt communication style to meet needs of patients, staff and colleagues, using tact, diplomacy and sensitivity including overcoming community barriers
  • To develop and maintain strong, transparent working relationships with key stake holders
  • To monitor and manage the team budget
  • To propose and implement departmental policy changes for the management of patients referred to the service, networking with other specialist practitioners
  • To represent the service and NCH&C at strategic meetings with partner organisations as requested
  • To manage the Community Access Team caseload
