This is an exciting opportunity to join a dynamic and creative community team working with young people across Cambridgeshire and Peterborough.

The Home Treatment Team work with 12–18-year-olds experiencing a crisis in their mental health wellbeing.

We offer a variety of interventions to support the young person, and their family/carers with the aim of this being within their community environment.

The successful applicant must be able to work both autonomously and as part of a team. You should be organised and able to prioritise accordingly.

The ability to drive is essential as this post requires the successful applicant to travel countywide.

You will have the opportunity to undertake further training within the Trust.

Main duties of the job include:

To offer support to young people and families at a time of crisis, when an admission to hospital may be considered, and with the hope that this could be prevented.

To provide ongoing support, therapeutic interventions when agreed, practical assistance and problem resolution under the supervision of a senior team member.

To contribute to the regular reviews of service users in the team.

  • To support the users of the Home Treatment Team with a range of practical, health and social issues under the supervision of a senior member of the team.
  • Work with service users to facilitate access and engagement in a range of community based activities.
  • Develop a rapport based upon championship and friendship, but within appropriate and transparent boundaries.
  • Help users gain access to resources including benefits and welfare rights.
  • Help to identify early signs of relapse by monitoring user’s progress, level of functioning and mental state and alert the appropriate staff involved in the individual’s care.
  • To attend meetings internal and external to the Trust when deemed appropriate by the senior clinician involved or Home Treatment manager.
  • Maintain adequate records as required by existing procedures, entering appropriate details on the service user’s case notes as necessary.
  • Attend and participate in team/care plan review meetings as appropriate.
  • Develop good working relationships with other organisations that are able to help individuals achieve their goals, including colleges, schools, other training providers and voluntary agencies, the inpatient units and community services

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.
