Job overview

The post-holder will oversee all administrative aspects of the TIA (Transient Ischaemic Attack) and Stroke Service at Homerton University Hospital NHS Foundation Trust.

In particular this  will mean: - Ensuring referrals are managed effectively and in line with set protocols  - Ensuring patients are seen in a timely manner and in line with set targets - Acting as an administrative resource to all members of the multi-disciplinary team  - Ensuring patient data is captured accurately and fully - Producing regular information reports for internal and external audiences - Liaising with all relevant service users - Appropriately managing all queries received by the service Comply with departmental and Trust procedures and guidelines e.g. incident  reporting - Comply with confidentiality and information sharing protocol

Main duties of the job

  • Contribute to the implementation of revised Stroke and TIA clinical care models as agreed with City & Hackney PCT.
  • Works  autonomously but in partnership with all members of the Stroke and TIA multi-disciplinary team and a central point for receipt of all referrals to the service.
  • Escalate any potential breaches and/or service issues to the Service Manager.
  • Manage patients through the relevant clinical pathways
  • Provide comprehensive administrative support to the multi-disciplinary team and facilitate all key service meetings .
  • Enter accurate data about patients onto Trust and department systems in real time produce information reports
